    Learn from your mistakes. Don’t be afraid to fail. We hear these statements so often that we’ve gotten used to them—in fact, they’ve almost become clichés. However, while we are aware that we should acknowledge and learn from our mistakes, the truth is that we often don’t.

    This failure to learn from mistakes is one of the greatest obstacles to human progress. As an example let’s look at the healthcare industry. In 2005, the UK National Audit Office revealed that 34,000 people were thought to have died in hospitals due to human error. Another study regarding UK hospital acute care found that one in ten patients are killed or injured because of institutional shortcomings or medical mistakes. Unfortunately, the healthcare industry does not routinely collect accident data. Without proper reporting, meaningful patterns cannot be established and people do not learn from mistakes. The result? A staggering number of unintentional injuries and deaths, despite incredible advances in modern medicine.
