    APPLICATION REQUIREMENTS Submit two sets of the following documents arranged and stapled as follows, no folder or envelope please

    (one set for HRM to be submitted on exam day and one set for the referring RMC/Branch's file copy): 1. completely accomplished 2012 Application for Employment Form with ID picture (refer to the attached Word/pdf file) 2. updated comprehensive resum 3. photocopy of transcript of records or true copy of grades 4. photocopy of diploma or certificate of graduation 5. photocopy of Civil Service-Professional eligibility or PRC rating (CPA, Engr, etc.) 6. photocopy of latest (not expired) PRC ID, if applicable PRE-EMPLOYMENT EXAMINATION SCHEDULE Venue: DBP Head Office, Sen.Gil J.
