ALS

( Amyotropic Lateral Sclerosis )

DEFINISI :
Adalah penyakit degeneratif pada motor
neuron ( UMN dan LMN ) di tractus corticospinalis, batang otak dan medulla spinalis.

INSIDENS
5 / 100.000 POPULASI Lama sakit 3 4 th ( < 1 th - > 25 th ) Umur saat onset dan tempat lesi,
menentukan prognose ( Bulbar > jelek dari Ekstremitas ). Faktor Genetik : - 10 % familial - 20 % mutasi gen Zn / Cu SuperOxideDismutase .

ETIOLOGI
TIDAK DIKETAHUI :
- Radikal bebas dan Stres oksidatif - Eksitotoksisitas - Sitoskeleton abnormal - Mekanisme autoimun

KLASIFIKASI
I. BULBAR II.SPINAL Bentuk klinis dan perjalanan sama ; 1.Degenerasi progresif UMN dan LMN, kelemahan progresif atropi otot lumpuh MENINGGAL ( Gagal nafas ). 2. Sensorik normal 3. Fungsi kognitif normal

GEJALA KLINIS
KHAS : Campuran UMN ( hiper refleksi )
dan LMN ( fasikulasi dan atropi ). - Bentuk I : kelemahan dan atropi otot2 lengan atau kaki disertai atropi tungkai. Bentuk II : kelemahan otot2 bulbar (disartri, disfagi ).

- Kelumpuhan makin berat, hiper refleksi,

fasikulasi, atropi eksterimitas. - Sensorik normal. - Kognitif normal.

 EMG : fibrilasi, fasikulasi, motor unit

polifasik, amplitudo tinggi dan durasi yang lama. LCS : tidak khas DARAH : serum Phosphocreatin kinase tinggi ( sel otot necrosis ) BIOPSI OTOT : jarang, kurang informatif

MANIFESTASI KLINIS
1. Sulit kancing baju ( atropi m.interossei ) 2.Jalan cepat lelah, kesandung ( drop foot ) 3. Ngiler ( parese otot menelan dan otot wajah ) 4.Atropi lidah, fasikulasi ( sering asimetris ) 5. Sulit mengunyah dan menelan 6. Sulit komunikasi lisan ( parese lidah, palatum molle, larynx )

PENATALAKSANAAN
Terapi kausal : tidak ada. Medikamentosa :
- Riluzole : mengurangi pelepasan glutamat
- TRH ( Thyrotropin releasing hormon ) - merupakan transmitter di kornu anterior - mempunyai efek tropik.

Simptomatik : quinin, sedatif ( spasme otot ) atropin ( ngiler ) Suportif : Fisioterapi

PROGNOSE
Meninggal 4 th setelah diagnosis Tipe bulbar, prognose lebih jelek
