CERTIFICATION EXAMINATION GUIDELINES 2012 (INDIVIDUAL) I. Application a. All applicants shall fulfill the following requirements to qualify for the certification examination: i. PRC License ii. Valid RENAP Membership ID iii. Valid PNA Membership iv. Valid IVT Card v. For RNs not working as dialysis nurses 1. Certificate of Employment / Clinical Experience with a minimum exposure of 6 mos. a. Clinical exposure in any nursing field where the applicant worked as registered nurse. b. Volunteer work shall be considered. c. For exposure in facilities where nursing practice is uncommon, a job description of the work shall be provided as additional requirement to ascertain whether this can be considered as clinical experience. 2. Dialysis training of at least 1 month. vi. For RNs already working as dialysis nurse 1. COE as dialysis nurse for 3 months 2. Induction / Orientation training in HD unit vii. Payment of Fees b. Application may be done online via the RENAP website or in person at the RENAP office. c. ON-SITE application is not encouraged. Only in special circumstances would an applicant be entertained for ON-SITE application. d. Upon careful screening of the submitted documents, the Certification Board / Committee shall provide the applicant a Notice of Confirmation via electronic mail to qualify for an interview with a RENAP Board of Director or an officially designated representative prior to taking the examination. i. The Notice of Confirmation provides the applicant the following information: 1. Time, date and venue of Interview 2. Time, Date and Venue of Examination ii. Confirmation may be deemed CONDITIONAL if the applicant is required to submit additional information. 1. Final approval to take the examination shall be upon the discretion of the interviewing RENAP official.

iii. The Notice of Confirmation shall be considered as the examination permit upon validation of the information presented during the interview by a RENAP official. II. Interview a. No applicant shall be allowed to take the examination without going through an interview with a RENAP official. b. The RENAP official shall ascertain and validate the information presented in the documents that the applicant submitted. c. The RENAP official reserves the right to deny or put on hold the application on the following grounds: i. Inconsistencies in the information presented ii. Dialysis training from unaccredited providers iii. Falsified documents iv. Failure to present the original documents despite submission of an electronic copy v. For CONDITIONAL qualifications where the applicant cannot comply with the submission of additional documents or requirements, deemed necessary by the RENAP official, within the time specified. d. The RENAP official shall then affix his signature in the Notice of Confirmation that shall serve as the Examination permit. Conduct of Examination a. The Secretariat shall prepare the number of test papers according to the approved number of examinees by the panel of interviewers. b. Preceptors shall be assigned to oversee the conduct of examination. c. In cases where the examination is to be conducted outside of Metro Manila, a BOD or an officially designated representative shall act as preceptor. No examination shall be conducted without the presence of the above mentioned officials. d. No examinee shall be allowed to go out of the room during the examination. e. Any form of cheating shall be a ground for the disqualification of the examinee and nullify the examination. f. All examinees shall start at the same time and shall not exceed the time specified to finish the examination as set by the Certification Board. g. If an examinee finishes ahead of time, he can submit his test paper and be allowed to go. h. Erasure in the examination is not encouraged. Examination Results a. Results shall be released within two (2) weeks after the conduct of the examination.

b. For conditionally qualified applicants, results will not be released until pending conditions are met. c. The passing rate shall be 75%. d. The secretariat shall provide a report containing the list of the examinees, the training provider of the examinee and the examination result. e. The Certification Committee Chair shall report to the BOD examination results on a regular basis. f. Any appeal in relation to the examination result shall be made in writing and addressed to the Certification Board. This shall warrant a review of the examinees test paper and a resolution shall be arrived at within a week upon receipt of the appeal. The examinee shall then be informed of the resolution. The resolution shall be deemed final and no succeeding appeals of the same case shall be entertained. V. QUALITY ASSURANCE a. All BOD shall submit on an annual basis a minimum of 50 questions to the Certification Board for pooling. b. The BOD shall then review and validate the questions prior to approval of use. c. A different set of questions shall be generated every examination schedule to prevent leakage. d. Only the Certification Committee Chair, President and the Secretariat shall have access to the final set of questions to be given every examination schedule. e. An examination set shall be printed a week before the examination schedule. f. In the event of excess printed copies, the secretariat shall make a report to the Certification Chair as to how many excess copies were made. These shall then be shredded and disposed.
