Steps to Planning and Facilitating a Quality IEP Meeting

Special Education Service Provider: __________________________________________ Student: ___________________ Date of IEP meeting: ______________ Team Members: ___________________________________________________________ Pre-Meeting: Provide families with adequate notice of meeting (5-7 school days) and establish a mutually agreeable time for the meeting. Provide meeting location and time to the IEP team in advance of the meeting (2-3 days). Gather data and evidence for use in writing goals and recommending services: Assessments from beginning of the year through now Anecdotals on class behavior, participation, strengths, weaknesses and interests SMART Goals, interventions, follow-up, progress and/or results Review IEP from prior year in order to suggest meaningful next steps in meeting goals Conference with student to receive feedback on what s/he feels has been working and what is still difficult Draft annual goals and recommendations based on data collected During IEP Meeting: Special Education teacher informs IEP Team how the student has progressed in meeting academic annual goals from previous IEP meeting - explicitly refer to goals and present level of performance General education teacher adds his/her input and gives feedback on student progress made in the classroom and how it pertains to special education services. Time is given for parent(s)/guardian(s) to share concerns; if student is present s/he may give feedback and share concerns as well IEP Team makes suggestions for special education services, testing accommodations and management needs based on data presented Sign attendance Post-Meeting: Special Ed Teacher Update IEP to reflect what was discussed at meeting Fax in attendance sheet to SESIS (print coversheet from SESIS) Send email update to teachers of the student with meeting notes Enter meeting notes and decisions on Skedula as an Anecdotal Teachers monitor progress toward goals Note interventions used to improve academics or behavior Collaborate with other teachers in order to offer student a consistent program of support
