Make the first appointment at the reception.

Receptionist keeps the referral and gives an
appointment card to patient.

Patient follow-up Patient follow-up with

report on request First assessment and treatment written by therapists

Make the following two appointments

(i.e. 2nd & 3rd) at the reception

Meanwhile, patient can only exchange

the appointment one for one. Special
arrangement should be in presence of
therapists note.
The last two appointments (i.e. 4th & 5th)
should be made at the end of the second
session.

Completion of 5 physiotherapy
sessions, copy and file the
physiotherapy referral and
progress sheet in patient record
