Guerrero St., Ermita, Manila, after having been duly sworn to in accordance with law, hereby depose and state: 1. That I am the registered proprietor of M HOTEL MANILA located at the address herein above stated and has been issued a Certificate of Business Name Registration by the Department of Trade and Industry which has a validity period from February 23, 2011 to February 23, 2016 with Registration Certificate No. 01324993 issued on February 23, 2011; 2. That I am requesting the Department of Trade and Industry to cancel the business name registration of M HOTEL MANILA; 3. That I have no financial obligation to anyone neither does the said business have any financial obligation to any creditor; 4. That I am executing this affidavit to attest to the truth of the foregoing facts and for whatever legal purpose(s) it may serve. FURTHER AFFIANT SAYETH NAUGHT.
