What to Expect at Your First Appointment If you are a new patient attending our clinic please take the time to come 15 minutes prior to your appointment to allow for time to fill out paper work. These forms are designed to provide the therapists at E.P.A. with detailed information pertaining to your health history and current condition(s). Your initial visit will involve a discussion with the therapist concerning the intake form provided. The therapist will use your information collected to perform a series of physical assessments to identify any impairments limiting your bodys ability to function. Please be advised that the assessment may take anywhere between half an hour to an hour, therefore this will require a 60 minute session and you may not receive treatment in this session. This is to ensure that the most effective treatment plan can be created for your for your next appointment. All services are rendered on an appointment basis. Please call to schedule an appointment. Please be advised that there is a 24 hour cancellation policy which is in effect and enforced at all times were a $40 charge will be added to your bill. If there is no cancellation with in 24 hours and a missed appointment the full amount of the appointment will be charged.
