DRUG STUDY

DRUG GENERIC NAME: Metoclopramide BRAND NAME: Plasil CLASSIFICATION: Antiemetic; GI stimulant DOSAGE: 5mg/ml;2ml FREQUENCY: OD ROUTE: IV

MECHANISM OF ACTION Stimulates motility of upper GI tract without stimulating gastric, biliary, or pancreatic secretions; appears to sensitize tissues to action of acetylcholine; relaxes pyloric sphincter ,which, when combined with effects on motility, accelerates gastric emptying and intestinal transit; little effect on gallbladder or colon motility; Increase slower esophageal sphincter pressure; has sedative properties; Induces release of prolactin

INDICATION

To control Nausea and Vomiting

CONTRAINDICATION Metoclopramide should not be used whenever stimulation of gastrointestinal motility mightbe dangerous, e.g., in the presence of gastrointestinal hemorrhage, mechanical obstructi on,or perforation. Metoclopramide is contraindicated in patients with pheochro-mocytoma because the drugmay cause a hypertensive crisis, probably due to release of catecholamines from the tumor.Such hypertensive crises may be controlled by phentolamine. Metoclopramide is contraindicated in patients with known sensitivity or intolerance to thedrug. Metoclopramide should not be used in epileptics or patients receiving other drugs which arelikely to cause extrapyramidal reactions, since the frequency and severity of seizures orextrapyramidal reactions may be increased

NURSING INTERVENTION 1. The nurse should assess the patient for N/V, abdominal distention, bowel sounds before and after, extrapyramidal side effects, tardive dyskinesia, and for signs of depression. 2. Give metoclopramide exactly as directed by your doctor 3. Give the missed dose as soon as you remember. However, if it is almost time for the next dose, skip the missed dose and take only the next regularly scheduled dose. Do not take a double dose of this medication. 4. Observe the client for severe side effects the nurse should know that this drug could alter hepatic function test results.
