Dear Sir, This is to advise you that Shri Shakil Zahoor Ahmad Zuberi who was residing at 23/B-306, Esteem Classic, Amrut Nagar, Jogeshwari (W), Mumbai-400102 has left with me your equipment, covered under your subscription voucher no. 24726 dated 16/12/2013 issued by Rangrai Agencies since ______________ Shri. Shakil Zahoor Ahmad Zuberi does not intend making use of said equipment, I request that the same may be transferred in my name and a fresh consumer number be given to me. Customers copy of the S. V. No. 24726 dated 16/12/2013 issued to Shri. Shakil Zahoor Ahmad Zuberi is surrendered herewith.

I confirm that

a. I am agreeable to pay to the cororation deposit for the equipment as per your terms currently in force. b. I do not posses a gas connection from any oil company.

I give below the consent from Shri. Shakil Zahoor Ahmad Zuberi request Bharat Petroleum Corpn. Ltd. to transfer his customership in my name and also authorizing me to sign the Termination voucher and accept refund of deposit amount on his behalf.

I, Shri. Shakil Zahoor Ahmad Zuberi hereby confirm that I have no objection to Bharat Petroleum transferring the subject gas connection covered by S. V. No. 24726 dated 16/12/2013 (Consumer No.: 28089) held by me to Shri. Mohd Tehsin Zuberi. I hereby explicity authorize bharat petroleum corporation ltd. to do so and to permit him to sign the T.V. as well as accept the refund of deposit on my behalf I hereby totally surrender my claim on this Gas connection and shall not have any claim whatsoever at any future date.
