    AFFIDAVIT OF UNDERTAKING

    I, GEMMA G. RABEJE, of legal age, Filipino, single and a resident of


    Bgy. Tiniguiban, Puerto Princesa City, Palawan, after having been duly sworn
    to in accordance with law, hereby depose and state:
    1.

    That I am a Bachelor of Laws student at the Palawan State


    University;

    2.
    That I took the subject CREDIT TRANSACTION twice, however, in
    both instances, I failed to pass it;
    3.
    That In order for me to be allowed to enroll it, for the third time,
    this second semester of SY 2013- 2014, I undertake to pass not only such
    subject but all subjects that I will enroll this semester;
    5.
    That I voluntarily and freely execute this affidavit to attest to the
    truth of the foregoing.
    IN WITNESS WHEREOF, I have hereunto set my hand this 14 th day of
    November 2013 in Puerto Princesa City.
