Generic name: Nifedipine

Indication : Treatment of high blood pressure
Drug Classification: Therapeutic category: Calcium channel blockers
Mechanism of Action: Inhibits Calcium ion from entering the “slow channels or select
voltage-sensitive areas of vascular smooth muscle and myocardium during
depolarization, producing a relaxation of coronary vascular smooth muscle and
coronary vasodilation; increases myocardial oxygen delivery in patients with
vasospatic angina.
Dosage: Adults and Elderly:
Initial: 10 mg 3 times/day as capsules or 30 mg once daily as sustained release.
Usual dose: 10-30 mg 3 times/day as capsules. Maximum: 120-180 mg/day
Special Precaution: Used with caution and titrate dosages in for patients with impaired renal
or hepatic function. Use caution when treating patients with congestive heart failure,
sick-sinus syndrome, severe left ventricular dysfunction, hypertrophic
cardiomyopathy, hypotension, second or third degree A-V block, concomitant
therapy with beta-blockers or digoxin, edema or increased intracranial pressure with
cranial tumors. Do not abruptly withdraw( may cause chestpain). Elderly may
experience hypotension and constipation more readily.
Pregnancy risk category C
Adverse Effects: an allergic reaction (difficulty breathing; closing of the throat; swelling of
the lips, tongue, or face; or hives), unusually fast or slow heartbeats, severe
dizziness or fainting, psychosis, yellowing of the skin or eyes (jaundice); or swelling
of the legs or ankles. Other, less serious side effects may be more likely to occur.
Continue to take nifedipine and talk to your doctor if you experience headache,
fatigue, or tiredness, flushing, insomnia, vivid or abnormal dreams, nausea or
constipation, increased urination.
Contraindication: hypersensitivity to nifedipine or any other calcium channel blocker and
adenosine.

Nursing Responsibilities: Monitor BP, heart rate, and I and O ratio closely when starting or
adjusting dosage. Monitor BP closely when used in conjunction with other antihypertensives.
Use orthostatic precautions for inpatients; advise outpatients about orthostatic precautions.
Monitor serum glucose closely in diabetic patients. Monitor for gingival hyperplasia.
