AFFIDAVIT OF APPLICATION
FOR EARLY RENEWAL OF PASSPORT


I, THERESA C. IBANEZ, of legal age, single and a resident of
No. 16, San Diego Street, Marulas, Valenzuela City, after being duly
sworn in accordance with law, hereby depose and state that:

1. I am a holder of a Philippine Passport with corresponding
number EB0536895, issued to me on 08 July, 2010, and
is valid until 07 July, 2015;

2. I was planning on applying/securing an employment in
Canada and one of the requirements thereof, is for the
applicant to be a holder of a Passport with a validity of at
least two (2) years.

3. Since the passport that I am currently holding is about to
expire in July 2015, or less than the required two years for
those seeking/applying for an employment in Canada, I am
constrained to apply for an early renewal of my passport.

4. I am executing this affidavit to attest to the truth of the
foregoing facts and for the purpose of complying with the
requirements of the Department of the Foreign Affairs for
securing an early renewal of my Philippine Passport.

5. I am also executing this affidavit for whatever legal intents
and purposes this may serve.
