CONSENT, AUTHORIZATION, RELEASE AND WAIVER

affilliates, su!sidiaries, representati"es, employees, licensees, successors, and assigns #collecti"ely referred to herein as NPR$
permission to inter"ie Child, to ta%e and use photographs, "ideo, film and&or audio of Child, and to pu!lish or otherise use any
image, li%eness or information o!tained a!out Child (including his&her name). 't is ac%noledged that these materials and this
information may !e used for any purpose, including !ut not limited to nes, information, pu!licity, promotions and campaigns,
throughout the orld on a royalty(free !asis in perpetuity and may !e edited or modified (collecti"ely, the NPR Content). The
undersigned parent(s) or legal guardian(s) understand and agree that they shall ha"e no right of appro"al, no claim to any
compensation, and no claim arising out of the use in any form of Child)s name, !iography, photograph or li%eness in the NPR
Content, and ac%noledge that NPR, in its sole discretion, may elect not to pu!lish any information a!out Child. The
undersigned parent(s) or legal guardian(s) agree that the material or information may !e pu!lished, !roadcast, disseminated, or
distri!uted in any format and in any media or system now existing or hereafter invented. The undersigned
parent(s) or legal guardian(s) agree that NPR is the sole and e*clusi"e oner of the copyright in the materials and may share,
license, sell or otherise e*ploit such materials in any manner it chooses, and that Child ai"es all right or control o"er any use
of the materials and information.
The undersigned parent(s) or legal guardian(s), indi"idually and on !ehalf of Child, here!y e*pressly release and ai"e any and
all claims (including !ut not limited to claims for li!el, pri"acy, false light, misappropriation of li%eness, right of pu!licity,
intentional infliction of emotional distress, or "iolations of federal and state pri"acy las) against NPR, and their respecti"e
agents, employees, officers, directors and assigns, in connection ith any material a!out Child collected or used as descri!ed
herein.
The undersigned parent(s) or legal guardian(s) state further that e ha"e read the a!o"e release prior to its e*ecution, and that e
fully understand its contents.
