AFFIDAVIT

I, BONIFACIO S. DEL VALLE, of legal age, Filipino, widow, and a resident of
Manguiring, Calabanga, Camarines Sur, after being duly sworn in accordance with law,
depose and state:

1. That I am a Geodetic Engineer with License No. 276177;

2. That I certify that the Approved Subdivision Plan that I prepared of Lot 2,
Psu-51204 covered by TCT No. RT-11950 (1918) containing an area of
490,623 sq.m as surveyed for the Heirs of Ladislao Perez and the designated
lots pertaining therein are the same as the lots covered by the Survey Plan
that I prepared for the Heirs of Ladislao Perez pertaining to Lot 1-A, Lot1-B,
Lot 1-C, Lot 1-D, Lot 1-E, Lot 1-F and Lot 1-G, all under Psd-05-00340 and
as well as those lots designated as Lot1-B,Psd-05-003340, Lot 1-A,
Psd 05-003340, Lot1-D, Psd-05-003340 and Lot1-C, Psd-05-
003340 as mentioned in the decision of RTC No. 26 in Civil Case No.
822, promulgated on August 10, 1983; and

3. As such, I am executing this Affidavit of Loss to attest to the truth of the
foregoing and for whatever legal purposes that this may serve.
