I, MARI MAR, Filipino, of legal age, married and with residence at Cebu
City, Philippines, after being duly sworn to in accordance with law, hereby
depose and state:

1. That I have retired as Teacher II in September 2012;

2. That I would like to claim my monetized leave credits for teachers with
the Department of Education for my unused leave credits;

3. That I have no criminal record nor have I committed any criminal
offense punishable by law;

4. That I am executing this Affidavit to support my claim for leave credits
with the Department of Education;

5. That each and every fact herein stated are true and I am willing to
testify before any court of justice to substantiate the truth of each and
all the said facts.
