Please read the policies and procedures carefully before accomplishing the online
application form.
1. Allot approximately 30-45 minutes to fill out this online application form.
2. Please make sure that you have the following information before accomplishing the online application form:

2. Parents and Guardian Information


o Full Name
o Birth Date
o Approximate Annual Income

3. Please note down the Application Number displayed at the upper right-hand corner of the online form. You will
need to provide this number in order to print the application summary.
4. Please supply all information requested in the application form. Take note that some fields are required to be
filled out before you can proceed to the next page.
5. Before clicking the Submit button, review carefully the information you supplied in all the fields. You will not be
allowed to make changes once the form has been submitted.
6. After submitting the application online, print the application summary (2 pages) on short bond paper.
7. Proceed to the Enrollment Services Hub (ESH) at 2/F Henry Sy Sr. Hall and submit the printed application summary
together with the required documents.
8. Applications are also accommodated at the following venues:


o DLSU Makati Extension Campus, RCBC Plaza, Tower 2, Makati City
o De La Salle University - Science & Technology Complex, Bian, Laguna

9. Deadline for submission of application is based on the applicant's Last Name. Please click here for more
information.
10. Pay the application fee and present the Official Receipt to the OAS to secure the entrance exam testing permit.
Your testing schedule will depend on the earliest testing date available upon the submission of complete
requirements.
