A. Project Overview: Describe the product or service of the project, the reason project will be
undertaken, and the purpose of the project. Discuss the problem or opportunity this project
addresses. Include the quality and safety issue this project will address. Support with current
evidence-based practice literature or data specific to the project.

I see a great opportunity to enhance patient education and retention on the progressive care
unit at McLaren-Lapeer Region hospital. I also see the need to offer diversions for pain
management and comfort. The focus of this project is to incorporate an education channel that
would discuss topics such as chronic obstructive pulmonary disease (COPD), heart failure,
diabetes, stroke, hypertension, end-stage renal disease and pain management. The addition of a
music channel would offer patients an alternative for pain management, anxiety relief and
agitation by creating a calm environment conducive to healing.
Consumer-centric health care, enabled by media and technology is integral to engagement,
self-management, behavior change and outcomes (Gruber, 2010). The prevalence of chronic
illnesses such as diabetes, heart disease and COPD is increasing and nursing is largely
responsible for promoting self-management. Health care reform, financial and staffing pressures
place constraints on nursing to provide adequate and thorough education to patients. Digital
media offers an adjunct to verbal and printed materials that integrate auditory, visual and
interactive learning strategies to increase knowledge transfer, recall and patient satisfaction that
accommodate various learning capabilities (Fox, 2009).
Music therapy has been shown to provide positive benefits by addressing spiritual,
psychological, physical, social, and cognitive needs of patients (Young & Koopsen, 2011).
Music therapy can improve emotional states, increase positivity, reduce anxiety, stress and
provide a tool for relaxation. Additionally, music has been shown to have positive effects on
physiological signs such as decreases in systolic and diastolic blood pressures and well and
respiratory rates (Korhan, Khorshid & Uyar, 2011).
The implementation of education and music channels can create an environment that
promotes retention of education, empowerment in self-management and a calm environment.
Increased patient satisfaction can increase survey scores which in turn increases hospital
reimbursement. Providing education that addresses various learning needs promotes patient
safety and improves health care outcomes. I have discussed this project with the unit manager,
nurse educator and patient advocate specialist.

B. Project Goal(s): Describe the project goal(s) using SMART (specific, measurable, accurate
and agreed to, realistic, and time bound) formula. These goals will be used to measure and
determine the projects success at its conclusion.
By the end of December 1, 2014, this project will:
1. Incorporate an education channel in patient rooms
2. Incorporate a music channel in patient rooms
3. Patients and nursing will report increased knowledge and retention with education provided as
reported on pre-post survey
4. Patients and nursing will report increased satisfaction in pain and comfort management as
reported on pre-post survey

C. Project Objectives/Deliverables: List the specific items or services that must be produced in
order to fulfill the goal of the project. Objectives/deliverables should be measurable results,
measurable outcomes or specific products or services. List and number in a logical order to
complete the project.
1. Gather information and evidence based research on the use of education and music channels in
health care, obtain list of distributers and cost of health education and music media (or cable
music channel) , develop pre-post evaluation survey materials by September 21, 2014.
2. Meet with McLaren-Lapeer Region management for input regarding implementation and cost of
channels by September 30, 2014.
3. Implementation of channels by November 1st, 2014.
4. Evaluate post implementation by use of surveys by November 14, 2014.
5. Meet with management regarding survey results and possible revisions in plan by November 21,
2014.

This is a breakdown of the objectives/deliverables into their most basic components. Consider
this the action plan of the project.

Assumptions:
1. research, cost, and materials will be obtained in timely manner
2. members of the team will be available at designated times and the project will move forward in
timely manner
3. the project will be easy to implement
4. cost of project will be within budget allowed by management
5. patients will express satisfaction with adjunct audio-visual education and increased comfort with
music therapy
6. nursing will be satisfied with adjunct educational options and music therapy for patient comfort
and pain management
7. current technological state of operating systems will support use of media
8. objective deadlines will be met
Constraints:
1. time
2. meetings and discussion times may not occur as planned- members of team may be absent
3. cost of project
4. operating system not able to accommodate media use
5. patients and nursing will perceive no real benefit of intervention

G. Success Criteria: Provide the value of the project. Why is this project important and of
what benefit it is? What impact will it have on client care and client outcomes including aspects
related to quality and safety? How will you know if the project was a success? Support with
evidence-based literature or nursing data.
The implementation of this project will be beneficial in improving patient health outcomes by
enhancing patient recall and knowledge retention through use of audio-visual materials and by offering a
calm and relaxing environment conducive to healing and pain management through use of music therapy.
Systematic reviews by McDermott & While (2013) demonstrate potential change in health behaviors and
clinical outcomes with use of digital/media based interventions. Currently, nursing at McLaren-Lapeer
struggle with time constraints in regards to patient education. While verbal and printed materials are
offered, audio-visual materials will add much needed reinforcement to current practice. Music therapy
provides a distraction that aids in pain management and comfort. Music also acts a means for spiritual
awareness and peacefulness (Young & Koopsen, 2011). Implementation of this option can allay the need
for copious narcotic use that promotes patient drowsiness and inattentiveness. An alert, non-drowsy
patient is better equipped to retain education for self-management.
I feel the success for this project will be observed in patient and nursing surveys. Satisfaction of
patients will be realized in hospital reimbursement and nursing can feel better about the care they deliver
to their patients.
