AFFIDAVIT STATE OF ALABAMA COUNTY OF MONTGOMERY My name is Kennedy Norman and I do solemnly swear the following is true and correct in every particular: Tam 23 year old male of African American descent, | currently have a residence address of 3941 Dallas Dr. Montgomery, AL 36105, which is where my mother resides also. She (Angela Wilson) has a cell phone number of 332- 0985. 1 am working on getting a new telephone today, but do not have it yet. ‘This past Friday evening, January 9, 2015, I was picked up by Montgomery police for unpaid tickets, I ended up staying in the jail until Monday evening January 12, when I was released and put on a payment plan. While I was checking out of the jail on Monday evening January 12, in the vicinity of approximately 10:00/11:00 p.m. I couldn’t help but observe a lot of commotion going on related to another inmate whose name was Tony Lewis. 1 ‘was checking out at the booth window in the center of the jail when I heard a seream, and then saw Mr. Lewis being grabbed and brought up to my area by two male guards and another male inmate, It was obvious that Tony was hurting. At this time I was placed in a special cell where all the mental patients were, but I had a clear view of what was going on. ‘Once Tony was brought up to the hallway area, I was only about five feet away and could easily observe everything that was going on. I was especially dismayed when I saw that one jail guard namely a big dude named Samuel Addy (who is African/A{rican) started wrestling with Tony. Then Addy (who is over six feet tall and appears to weight as much as 325Ibs) placed a knee on the back of Tony's neck because Tony was moving too much, and then placed another knee on Tony’s arm, ‘At that point a nurse (who is a Caucasian lady with red hair) came walking up quickly and attempted to give Tony a shot to calm him down. However, Tony ‘was moving too much and therefore she quit trying and claimed that Tony was faking Meanwhile, two black female guards appeared and said that there was something, wrong with Tony and that he wasn’t faking. I also yelled out something is wrong. Thereafter the white female nurse left. ‘The big Affiean guard, Samuel Addy, continued to have his knee on Tony’s forearm and another knee on his, neck while holding his arm down with his hand.10. Te 12. 13, At this point a black female nurse came up and told the guards to sit Tony up. ‘When they tried to sit Tony up, Tony appeared to give out. That is he wasn’t moving at all. I saw a black nurse check for a pulse, but there didn’t appear to be apulse, Again, ] was only five feet away and could easily see and heat what was going on. ‘After that I saw a black nurse say to the guards to lay ‘Tony down on his back. At this point, the nurse and guards were attempting to do CPR and Tony appeared to move one arm and his head a little, but he had blood coming out of his mouth and was groaning. Shortly thereafler, I saw Tony’s head drop as if he were ‘unconscious, ‘Meanwhile there was a black, who was the Sarge, who kept walking back and forth between a room where he was watching a game on TV and the place where Tony Lewis was lying. I then noticed that a black nurse started doing CPR on Tony again. When Tony didn’t seem to be responding, I heard her tell the black female guards that “y'all should call the ambulance; you should have already called it.” 1 observed paramedics arrive and I heard one of them say that “I have lost him again,” as Tony was being picked up and put on a stretcher. I also noted that ‘when the paramedics arrived, the red headed nurse acted like she was concerned at that time and trying to bring Tony back, [finally got checked out of the jail at about 11:00 p.m. at which point I walked ‘outside the municipal court building on Ripley Street and saw two women, one of ‘whom was crying a lot. It tumed out to be Tony’s aunt, Harriett Mims, who had accompanied Tony’s mother, who was inside the building. I felt the vibe that they were there in connection with Tony and I proceeded to tell Ms. Mims some of what I had observed and have shared above.
