HIGH POTENTIAL INCIDENT IN AHTS VESSEL
O QUE ACONTECEU:
WHAT HAPPENED:
Rompimento de cabo de polister do sistema de ancoragem durante o pr-lanamento causando
liberao do sistema em rea congestionada. Os elementos submarinos no foram atingidos.
Rupture of the Anchorage system Polyester rope during pre-launching causing the release of the system in a
congested area. The subsea elements were not affected.

O cabo possua danos pr-existentes, e

mesmo assim, o supervisor responsvel
pela operao decidiu utilizar o cabo;
Even though there were previous damages on the
rope the supervisor in charge of the operation
decided to use it.

Falta de conhecimento de procedimento

de passagem de cabo pelo rolo de popa,
que determina que o mesmo deve ser
molhado para que o atrito da passagem
pelo rolo seja diminudo.
Lack of knowledge of the procedure to pass the
rope through the stern roller, which says that it
should be wet to reduce the friction while passing
by the roller.

O QUE DEVEMOS FAZER PARA EVITAR:

WHAT WE SHOULD DO TO AVOID:

Revisar e divulgar os critrios para utilizao e descarte de cabos quando forem percebidos
danos.
Revise and disclose the criteria to use and discard ropes when damages are found.

Treinar as tripulaes das embarcaes da empresa contratada no padro de Operao de DMA

enfatizando o correto acondicionamento e preparo do cabo no guincho, de forma que seja
sempre molhado antes de sua passagem pelo rolo de popa.
Train the crew of contractor company vessels on the standard of DMA operation (mooringunmooring/Handling operation), highlighting the correct stowing and preparation of the rope on the winch,

so that it is always wet before passing through the stern roller.
