WHAT HAPPENED:

Durante mergulho noturno, em manobra de retirada de pea submersa no apoio atividade de pull
in, o mergulhador teve o seu dedo prensado pela pea quando a mesma estava sendo iada pelo
guindaste com auxlio do cat line.
During a night diving while giving support to the pull in activity to remove an underwater part, the diver had
his finger pressed by the part when it was being heaved up by the crane with the help of cat line.

O QUE DEVEMOS FAZER PARA EVITAR:

WHAT WE SHOULD DO TO AVOID:

Aprimorar a sistemtica de treinamento e divulgao dos procedimentos operacionais rotineiros e

no rotineiros nas frentes de trabalho;
Improve training system and disclose operational procedures routine and non-routine at the work fronts.

Revisar as APRs e os procedimentos contemplando os cenrios acidentais de impacto de objetos

e prensamentos durante a movimentao de objeto submerso.
Revise the PRAs and the procedures including accidental scenarios of objects impact and pinch points while
handling underwater object.

Considerar as condies de visibilidade e mergulho noturno nas avaliaes de risco;

Consider the visibility condition and night diving on the risk assessments.

Realizar treinamento para as equipes das frentes operacionais, considerando os riscos inerentes
movimentao de cargas submersas e os cuidados em relao aos perigos potenciais.
Carry out training to the operational work front teams, considering the existing risks of underwater cargo
handling and attention to the potential dangers.
