HIGH POTENTIAL INCIDENT OCURRED IN AHTS
O QUE ACONTECEU:
WHAT HAPPENED:
Perda de propulsores com risco de abalroamento. Devido deficincia no lubrificador de cilindros, por
falta de trava qumica dos parafusos internos da base da lubrificadora.
Loss of thrusters with risk of shock. It happened because there was deficiency of cylinder lubricator caused by the
lack of adhesives of internal screws of the lubricator base.

Incluir a inspeo da rede de comunicao no gestor de manuteno;

Include the inspection of communication network in the maintenance manager;

Incluir a inspeo dos parafusos internos da lubrificadora;

Include the inspection of internal screws of the lubricant;

Inserir a trava qumica na manuteno dos parafusos internos da lubrificadora;

Insert the adhesives in the maintenance of internal screws of the lubricant;

Inserir a trava qumica na manuteno dos parafusos internos da base da lubrificadora no gestor
de manuteno;
Insert the adhesives in the maintenance of internal screws of lubricant base in the maintenance manager;
