Benchmark: Socrative Web 2.

0 Tool- Quiz on Healthy Eating


Measurable objectives:
o Listed in the lesson plan; because this is a benchmark
assessment, students do not necessarily have to get a certain
percentage of the questions correct. This assessment would be
used more to help me, the teacher, understand where students
are at and how much they already know. I will use this
benchmark to group students for the lesson.
Directions for completing the assessment:
o This assessment was made on the Socrative tool that can be
downloaded onto any system (iPad, computer, etc.) I will use this
quiz at the beginning of the class. Each student will have access
to the quiz and will be able to go at the students pace. Students
will also have the option to get immediate feedback and
explanations after they answer each question. The students will
answer the questions in order and cannot skip or change their
answers. As the teacher, I am able to monitor progress for each
student as they are completing the quiz. To accommodate certain
students, I may read the questions along with the answers, aloud
to either a small group or for the entire class.
The task outlined:
o This benchmark will assess how much the students already know
about being healthy. Students will be grouped based on how
much they already know and then even more scaffolded based
on which type of information they know and do not know.

Directions for completing the assessment:


o Students will complete the data notebook sheet as we move
through the lesson and touch on each food group. This is a unit
lesson that would cover one-week time. Each day, we would
address a new food group. Students should fill in their data
notebook sheet as we move on. Students may also edit their
data notebook sheet to include more columns or other
information they may need to.

Summative: Group Project- Meal and Exercise Plan for One Week
Measurable objectives:
o Listed in the lesson plan; this summative assessment will allow
the students to think creatively and create a final project using
whatever they would like to present (paper, poster, project, book,
computer).
Directions for completing the assessment:
o Students will be assigned groups of 3-4 and given allotted time in
class to work on the final project. Each group will be creating a
meal and exercise plan for one week (7 days) for their
experiment student.
The task outlined:
o The plan should include:
Breakfast, lunch and dinner meal each day; foods must
identified under the correct food group
no meal should be repeated
Exercise activities for at least 30 minutes each day
Instructions for how to complete the exercises
Amount of sleep each night
Description of the experiment student
Presented creatively to the class
