AFFIDAVIT OF UNDERTAKING
I, AILEEN BODEJAS, of legal age, Filipino, and residing at Brgy. Salacay,
Cabatuan, Iloilo, Philippines, after having been sworn to in accordance with law, do
hereby declare and state:
1. That I am the mother of the minor AXL ROSE BODEJAS, who was born out of
wedlock on 07 June 1997 at Cabatuan, Iloilo, Philippines;
2. That I am applying for the issuance of a passport for my minor child AXL
ROSE BODEJAS with the Department of Foreign Affairs, Iloilo City office;
3. That due to inadvertence, the birth certificate of my son, AXL ROSE
BODEJAS, states that his gender is FEMALE when in truth and in fact, he is MALE;
4. That I undertake to have the discrepancy corrected in court as early as
possible and have in fact filed a Petition for Correction of Entry now pending before
Branch 28 of the Regional Trial Court in Iloilo City under Special Proceeding No.
07-8787;
5. That I will submit a certified true copy of the court decision on correction of the
discrepancy entry on gender, together with the birth certificate bearing the annotation of
the said court decision, if and when I apply for renewal of the passport of my son; and
6. That I am executing this affidavit to attest to the truth of the foregoing facts and
for the purpose of applying for the issuance of a passport for my son.
