Physician Incomplete/Delinquent Record Process

1998-2001: Upper Valley Medical Center

Worked one on one with physicians to complete their records


Printed incomplete lists for each physician then contacted their office manager to notify them of their delinquent charts
Focused on keeping the physicians compliant to avoid their suspension
Removed deficiencies and returned completed charts to coders if needed, and filed back completed charts in the main file

Coding

Worked the AR report for inpatient/observation/same day surgery to acquire complete physician queries
Responsible for ABNs on outpatient testing
Worked with offices to justify test if ABN was not present on chart
Keyed charges for outpatient coding
Follow up for re-occurring accounts (physical therapy, speech therapy, etc.)
Created ABN reports quarterly for meetings
