A physical therapy department is vital for this tri-county area because there are no resources
available that serve these people. The department will help people in this rural area to achieve mobility
and quality of life. Physical therapy is an important step in the recovery process and prevention of
further injury. Our philosophy is to assists each individual achieve their own optimum state of health
and this is our core purpose as physical therapists. It is our goal that all patients are deserving of
respect, dignity and recognize that everyone has a unique culture and social belief and values.
3. Two PT's will work every day Monday through Friday. There will be two PTA's to every PT, 4 PTA's
total. A PRN will and a part time PTA will work Saturday's. The clinic will have a space provided by
the hospital at a discounted price of $10 a square ft. Patient referrals will come from the surgeons and
doctors in the hospital to keep the clinic fully running and connected with the hospital patients.
4.There will be people recovering from all types of surgery procedures, and injury that come straight
from the hospital, customers will come by doctor referral due to the clinic being the first and only clinic
in the area for a long period of time. The types of physical therapy delivered will be aqua therapy,
cardiac rehabilitation, inpatient & outpatient rehabilitation, and wound care.
5. Identify resources necessary to achieve goals:
a. Personnel: To start out the business, staff will include: 4 PTA's that have graduated from an
accredited school, and 3 PT's that have a doctorates in the practice of physical therapy, 10 Aides, a
bookkeeper, a secretary and a PRN that comes in for extra support.
b. Materials and disposable supplies required for operation: towels, pillow covers, bags for the
trash, tissues, theraband, and ultrasound gel.
c. Pieces of major equipment in order to operate the department: Chairs for a patient waiting
area with a table of magazines, a small pool is included in the space for the clinic, exercise equipment
such as treadmills, weights, stationary bikes, exercise tools for the pool and cleaning equipment for
pool maintenance, hot and cold packs, an ultrasound machine, estim machines, grastid tools etc.
d. Small donations will come from from KT tape in for us to use their product and educate patients of
at home use of the tape. The Board of Trustees has saved up 45,000 to start the clinic off, in dire need
of physical therapy. Revenue from then on out will be gathered through patients.
6. The PT department will be open Monday-Saturday. There will be two PT's and a PRN PT that helps
out for vacation and sickness comes in on Saturday. Each PT is required to work 40 hours a week.
There will be four PTA's working Monday-Friday 8-5 working 40 hrs a week and a part time PTA
working Saturday's and where needed. A PT secretary will take care of scheduling the inpatients and
outpatients, and will create the work schedules. Each inpatient will receive a 40-minute morning and
afternoon session as needed. Aides are part-time jobs working no more than 27 hours a week.
7. Job description:
a. Director of Physical therapy directs physical therapy services by developing strategic objectives;
providing services. Develops physical therapy organizational strategies by contributing information,
analysis and recommendations to strategic thinking and direction; establishing functional objectives in
line with organizational objectives.
b. PT: This position is responsible for screening, testing, evaluating, diagnosing and treatment of
injuries, diseases, and disabilities using physical therapy procedures and modalities in accordance with
standard physical therapy practices; and also consulting, educating and training patients and their
caregivers.
c. PTA: The Physical Therapist Assistant (PTA) partners with a licensed Physical Therapist in
providing care and treatment to Physical Therapy patients. The PTA is a valued contributor who
participates in a direct care role and is responsible for developing a healing treatment plan and
implementing physical therapy treatment procedures, therapeutic activities and life skills training.
While delivering quality care a Physical therapist Assistant acts in accordance with state licensure, laws
professional standards and ethics and is committed to providing extraordinary care to the patients they
serve.

d. Aide: The role of the Patient Service Rep (PSR) is to provide extraordinary care to patients and
customers through friendly, caring, courteous, and professional services. A successful PSR becomes the
patient's advocate and works to facilitate resolution of patient accounts by working with insurance
companies, government agencies and patients in the payment for services that have been provided. The
PSR has the opportunity to use their valuable skills and resources by responding to questions,
researching discrepancies and insurance denials and following the hospital policies and procedures for
billing and account management.
The PSR/PT Aide has a variety of office responsibilities in which they are contributing to a friendly and
helpful office environment. This Position will warmly greet and assist patients, answer phones,
schedule appointments, file, send bills and maintain patient records.
This position will also act as a PT Aide. The Physical Therapy Aide is a member of the healing team by
educating patients on exercise and equipment usage, applying or removing braces and/or wraps,
preparing patient rooms, stocking supplies and operating exercise equipment.
8. Average evaluation price will be $65, and the intervention price will be $50. The average number of
appointments in a day would be 10-15 and average 5 no shows.
Expendables would include the lost money when there are no shows, towels, cleaning supplies. In
limiting the expendables, no more than one towel will be used per patient visit and only if needed, the
minimal amount of water and bleach will be used in the laundry to limit the utility bill.

VI. Dress code: Close toed shoes are acceptable, modest clothing (no bare midriff and cleavage), PT's
dress in khaki or dress pants with a button up shirt or polo, PTA's must be in royal blue scrubs, no
tattoos showing, no inappropriate or abnormal hair color, no distracting/dangerous piercings.
10. The director of the Physical therapy department will pick two goals for the employees to achieve in
a year that will each be weighed at thirty percent. The staff will choose two of their own individual
goals each weighed at twenty percent that they would like to achieve. The goal must be SMART goals.
Specific, Measurable, Results-Focused, and Time-Bound. To keep customer service top notch, each
member of the staff will have an instructional video applying to their job description every month.
