SUPPLEMENTAL AFFIDAVIT OF LEGITIMATION

I, WILSON L. JUMAWID, of legal age, married, Filipino citizen
and a resident of Bambong Diut, Ramon Magsaysay, Zamboanga del
Sur, after having been duly sworn to in accordance with law, do hereby
depose and say;
1. That I am legally married to Marie Ann U. Mendaros at a
wedding ceremony solemnized by Rev. Fr. Romeo Hontiveros
on 17 March 1998 and said marriage was duly recorded at the
City Civil Registrars Office of Pagadian under Registry No. 98504. A copy of our marriage certificate is attached hereto and
made
an
integral
part
of
this
affidavit.
2. That our child, WILMER MENDAROS who was born on 07
March 1988 and his birth was registered on 05 November
2004 at City Civil Registrars Office of Pagadian under Registry
No. 2004-5390;
2. That at the time of the conception of said child, we were not
disqualified by any impediment to marry each other, and,
therefore, by virtue of our subsequent marriage, the said child
is now legitimated by operation of law, particularly Article 177
of the Family Code.
3. That I am executing this affidavit in order to attest the truth of
the foregoing facts for the LEGITIMATION of our child.
