Ratings are distinct for each semester; they are not cumulative. Under each competency, specific practice
behaviors are listed, along with specific measures for evaluating student performance.
This form may be personalized by adding comments after each section. If any item within a section has
been rated either Exceeds, Does not meet or Marginally meets Expected Competency, a specific

The Rating Scale is:
(4) Exceeds Expected Competency: The student demonstrates knowledge and ability above the
criteria set for each practice behavior.
(3) Meets Expected Competency: The student demonstrates both ability and knowledge which meet
the basic expectations as listed in the criteria for each practice behavior.
(2) Marginally Expected Minimum Competency: The student requires intensive redirection. The
student demonstrates effort but has difficulty in achieving required expectations for each practice
behavior.
(1) Does Not Meet Expected Competency: The student is unable to perform even minimal tasks and
does not demonstrate an understanding of the theoretical base of materials.
(0) Not Applicable/No Opportunity: Student has little or no opportunity to engage in task.

Comments: Kelly is very good at recognizing the importance of appearances with guests. Her
attendance was fine but she did not always do her hours that date. She was able to make them up
because she started in November 2014 with the orientation. She is always determined to do what is
correct.

Comments: Kelly has high ethics and this has assisted her in working with the guests and in this small
non profit with constant change. There has been change in management and staffing at the time of
opening for the season. She was able to navigate without making any ethical mistakes.

Comments: Due to Kelly's employment outside of Welcome Inn she has practice in good professional
writing. Her inquisitive mind also allows her to organize her thoughts on what would be a individual
plan for the guests. She demonstrated that she understood how to use her personal experience when
interacting with the guests.

Comments: Kelly is a life long learner and has demonstrated that going back to school and looking at
life through the eyes of the guests. She demonstrated the ability to connect with the guests and become
that person that they felt understood them.

Comments: Kelly demonstrated her understanding of the many forms of oppression faced by the guests
with her interactions with the guests. She advocated well for the guests to obtain services and find
family so that they were not alone.

Comments: This was more of a crisis program that did not allow much time for research.

Comments: Kelly demonstrated good knowledge of methods to obtain information from the guests and
being able to obtain resources for guests.

Comments: Having Kelly at Welcome Inn aided in the development of good policy and procedures due
to her questions and concerns. Since we were all new to Welcome Inn including the Director, it was
good to have her question how the services provided effect the guests.

Comments: Due to the crisis mode of the work place, Kelly did not get to be involved with a lot of
external advocacy but she did participate in the Point In Time count of homeless people in Oakland
County. She worked with the organizations that came into to help the guests like paperwork for housing,
social security disability and supplemental, and employment.

Comments: Kelly followed having scheduled and unscheduled meetings to discuss guests and structure
and resources. She is great at learning information and using it to aid guests in connecting resources.

Comments: Kelly openly expressed her concern that she would not be able to end her professional
relationship with the guests. She understood that she would not be able to see the individuals receive
services until they were housed. We discussed at length so that she could process her farewells.
Kelly is good at working with guests to open up especially about family matters. She worked hard to
unite families during her intake and followups.

Comments: Kelly is very good at wanting to be great at her work. She is also an inquisitive person and
that allows her look at how a service is provided and what would make it better.

1. Is beginning to consistently recognize the challenges within an urban environment and the impact
of these on client systems

Comments: Kelly also works in an urban hospital which allows her to understand the challenges that
the guests had and to overcome.
