AFFIDAVIT OF LOSS OF SCHOOL ID

I , Mark Justine O Borreros , Filipino, of legal age, and a resident of

Mapulang Lupa Valenzuela City ,after having been duly sworn in accordance
with law, hereby depose and say:
1. That I am a holder of a school identification card bearing student number
2010-45953 issued by UP Manila.
2. That I used to put said identification card in my bag
3. That sometime on June, when I arrived home from school, I discovered
that my school ID was no longer in my bag where I used to place it;
4. Despite diligent search and efforts to locate said ID, I could not find it.
5. That I am executing this affidavit to attest to the truth of the foregoing
facts and to request for are placement of said School ID from UP Manila
