AFFIDAVIT OF DISCREPANCY
I, CAVIN KAY DE GALICIA RIGOR, 21 years old, Filipino, single, and a resident
of No. 7-C Trinidad Street, Barangay Gulod, Novaliches, Quezon City, after having been
duly sworn to in accordance with law, depose and state THAT:
1. I was born on the 7th day of November 1992, by spouses Jane L. De Galicia
and Wilfredo D. Rigor;
2. I have been using the name CAVIN KAY D. RIGOR in all of my scholastic
records and other documents;
3. However, I honestly and wrongfully, because of my negligence, indicated my
middle initial as DG instead of D in my application for eligibility exam for Civil
Services;
4. I passed the 2013 Civil Service Eligibility Examination in the level of
Professional, and the Certificate of Eligibility expressly provides my middle initial as
DG instead of D;
5. The DG and D refers to one and the same middle name for DE
GALICIA, and CAVIN KAY DG. RIGOR and CAVIN KAY D. RIGOR refers to one
and the same person; and
6. I execute this Affidavit of Discrepancy of Middle Initial to attest the truth of the
foregoing facts and explanations in my application form in the said Examination for
Eligibility in Civil Service and also found in the Certificate of Eligibility for Civil Services.
