Definition of First and Second party

The purpose of the agreement is 5 clergies of Puja Mandala gives spirituality service for

BIMCs patients
Article 1: Puja Mandala provides 5 religions : Islam, Hindu, Buddha, Catholic, Christian
Article 2 : BIMC Hospital is allowed to get spiritual service any time which appropriated to

patients need and condition.

Article 3 :
1. The service : Prayer for patient and family, and prayer for the end of life for terminal
condition
2. Time : approximately 30 minutes for visit
3. Time of service is adapted with patients needs
4. Fee for each visiting is 200,000 Rp
