Cooperating Teacher's EVALUATION OF FIELD OBSERVATION STUDENT


Please complete the following evaluation (WITH THE STUDENT) after he/she has
completed their observation requirement. Use the Performance lndicators Rating
Scale provided to rate the student on the Performance lndicators that have been
listed. Your candid, honest assessment is very valuable to the student's future
development as a prospective teacher. Also, please comment upon the student's
strengths and/or weaknesses you may observe. As a seasoned professional, the
student looks forward to, and appreciates your constructive comments.
