-I utilize my knowledge about human

growth and the various life transitions
one goes through to be of better support
for my clients as they go through this
development and transition to
motherhood and birth.
-I engage my patients in identifying their
health needs and goals by explicitly
asking them and listening to them about
their concerns, goals and thoughts
about their health, healing and care. If
the patient identifies something of
concern of has a specific need, I try to
the best of my abilities to meet these
needs to make their stay at the hospital
as great and helpful as it can be.
-I utilize knowledge learned in my NURS x
2003 about the pathophysiology of
diseases when looking for signs and
symptoms of various diseases or
conditions in my clients, as well also to
simply have a better understanding of
what these specific diseases are and
how they affect the body.
-I utilize the knowledge from my NURS
2001 class about relational inquiry to
better relate and provide client centered
care. From this course, I specifically
utilize the relational capacities and skills
learned to enhance my relational
connections with clients. Such skills

3. Critically appraise relational inquiry processes

and skills to develop meaningful relationships
with health care providers and family
members.

4. Collaboratively formulate a plan of care based

on knowledge of family nursing,
related theories and scholarly literature.

include the skill of listening and

capacities include initiative (reaching
out and the desire to help the client
through this time) and also authenticity
which I demonstrate by always being
real and humanly present with all
patients when they are in my care.
-When providing care, and discussing
health topics with patients, to the best
of my ability, I minimize the potential for
my own personal values, beliefs and
position to influence any of my
responses/actions and also to minimize
the potential for biases. When doing so,
I also remain professional while being
culturally competent and respectful of
other cultures and religions (example:
one week I had a family of different
cultural background who had a different
approach to various things involving the
care of the baby and communication,
with this I simply changed by plan of
care a little bit to better accommodate
and suite their needs to provide better
and culturally appropriate care.)
-At every clinical placement day, I carry
out the care of my assigned resident
according to the institutions policies,
therefore, I carry out vitals and
assessments every four hours (or as
needed depending on patients
situation) and adjust any additional care
based upon findings (whether abnormal
or not) and respond appropriately.
-In labour and delivery I followed the
policy of care for mothers in labour and
did check ups on mothers every fifteen

5. Develop, implement and evaluate the

effectiveness of health-promoting,
evidence-based practice, reflecting principles
of family nursing as relational
practice.

6. Demonstrate increasing competence and

confidence in the application of
psychomotor skills in practice settings.

minutes or as required for their

individual status/condition.
-I am aware of where to find resources
and supports if I, or another student, are
in need of assistance. I know where to
find specific policy, procedure and
protocol manuals on the unit to ensure I
am providing safe and evidence-based
practice.
-During every assessment I ask and
inform to ensure clients have proper
basis of knowledge about not only
newborn care but also their own care for
both in the hospital and in the home
environment. I cover topics such as
breastfeeding, sleeping positions (for
baby), peri care, newborn bathing,
newborn diet, birth registration, health
card information and so on to ensure the
patients have all the knowledge and
resources they may need before leaving
the unit.
-I utilize the appropriate assessment
tools and techniques as according to the
skills and procedures learned in clinical
labs and as mandated by the unit
policies in order to provide the best care
possible.
-Whenever needed, I consult other staff
members or my clinical instructor about
any uncertainties regarding skills,
techniques and procedures.
-Safety is a priority at all times for
patients, colleagues and myself. Safety
is prioritized by utilizing appropriate
personal protective equipment (gowns,
gloves, mask, etc. as needed), constant

7. Demonstrate accountability and

professionalism that is consistent with a nurse
entering a self-regulating profession.

8. Select appropriate community support

services for families needing referral to
enhance coping with diverse transitional
experiences.

hand hygiene, using Cavi-wipes, wiping

down equipment such as vitals
machines/stethoscope, putting up
bedrails, placing newborns in isolettes
properly, and holding/handling babies
properly, to name a few.
-I demonstrate accountability by being
at placement on time and prepared for
the day and also perform assessments
on my patients in a timely manner as
directed by unit procedures and my
nurses instructions.
-I organize my workload by planning out
my assessments and when I will do
certain tasks throughout the day (give a
demo bath, bili meter, going over
information package, etc.) in order to
have good time-management and also
fulfill all my duties.
-I understand that there are certain
tasks that I cannot do as a nursing
student or that I do not have much
experience with thus always ask for
assistance if such things arise. (An
example of this is in labour and delivery,
there were things that I could not do
such as change IV bags and also I was
not very experienced with reading the
fetal monitors and such therefore I
asked for assistance from the other
nurses in the unit.)
-With the patients I am assigned to
(especially those being discharged soon)
I go over the information package
provided by the hospital about
community resources, health card
forms, birth registrations and tips for

new mothers (ex. Breastfeeding, when

to give solid foods, sleeping habits,
etc.).
-I answer any questions parents may
have, such as any questions about
bathing or proper sleeping positions for
the baby and where they can get more
information on such topics if ever in
doubt in the home setting.

Student Areas of Strength

1. Communicating with clients effectively, making them feel comfortable and at ease and building
therapeutic relationships with clients.
2. Maintaining and calm and professional attitude at all times in the workplace, especially in the
presence of clients and other staff members.
3. Performing newborn assessments in a timely and effective manner, without having to return to
the clients room a multitude of times because I forgot to certain assessments and also feeling
confident and accurate in my findings. As well as being able to detect what is a normal finding and
abnormal finding requiring supplemental attention.
Student Areas for Future Development
1. Continue to practice and review more advanced skills learned in later learning labs such as
catheter insertions/removals, IV prepping, and injections.
2. Practice writing proper and relevant SOAP notes for charting purposes.
3. Learn more about breastfeeding to be a better resource for mothers when they arise with
questions, concerns or challenges with this, as currently, I have a very basic knowledge about this
topic and would like to learn more to better educate and be of better assistance.
