Extracentricular Neurocytomas: Pathologic Features and Clinical


Outcome
Introduction:
Brain tumors are abnormal cell growth with in the brain. Specifically, extraventricular
neurocytomas are atypical nerurocytomas. These lesions are typically low grade, along with a
fairly high survival rate. Extraventricular neurocytomas (EVNs) are usually located in the
ventricles of the brain. The ventricles of the brain helps store the cerebral spinal fluid (CSF).
Tumors located in the brain ventricles, cause a high risk of death due to the pressure on the brain
from the tumor blocking an easy flow of CSF.
The Neurosurgeons wanted to see the pattern and growth rate of extraventricular
neruocytoma (EVN). Doing the experiment allowed doctors and researchers to figure out how to
treat the tumor. Scientists wanted to figure out how to treat the random occurring brain tumors.
Scientists wanted to see the growth rates, possibility of recurrences, survival rates, along
with what treatment was best for the extraventricular nerurocytomas. Doing the research help
save lives along with find treatments and are currently trying to solve how to prevent this from
occurring.
Extraventricular neurocytoma is a rare tumor that causes hydrocephalus (swelling in the
brain). The swelling can lead to coma or even death if not treated properly. These tumors only
started occurring in 1992, and is still very uncommon.

The researchers hypothesis showed out of the study of 35, some of EVNs did not
regrow. With the majority of tumors, not all of them were 100 percent gone, but the tumor
became harmless and stable afterwards. Out of 30 cases that include a follow up, 10 tumors
reoccured, three of those caused the patients to die. After the second set of surgery, or radiation,
the tumor did not regrow at all.

Materials and Methods:


The experiments consisted of 35 patients who at least underwent brain surgery once. 15
of the 35 patients after surgery, had radiation therapy. All the patients ages ranged from 5 years
old to 76 years of age; both male and female. Many of the patients underwent one brain surgery
and had a biopsy of the tumor. Monitoring was required after the surgery. Monitoring consisted
of MRIs every three months for two years, one MRI every six months for one year, and then
after that one MRI on the fifth year. The procedure repeats or starts over if the EVN reoccurs.
Surgery techniques consisted of open brain surgery and sometimes combined with
radiation following after the surgery. Two types of surgeries were performed: gross total
resection (GTR) or subtotal resection (STR). With the GTR, the whole tumor was taken out,
while with the STR only part of the tumor was taken out due to a complication that would put the
patients life at risk. Radiation doses consisted of 5400 cGy to 6490 cGy. If found alive with
disease (AWD), many did not undergo anymore treatment.

Results:
Five of the 35 patients results were unable to follow-up with the rest of the treatment and
research. The results showed that only three of the patients died. Those three patients were over
the age of 67, all being female. 12 of the 35 patients had no evidence of disease (NED). 15 of the
patients had a returning tumor with as soon as 6 months. The majority lived with the tumor
AWD.
Many researchers were surprised but received with the few deaths. But many patients are
not able to handle a serious surgery. But the many reoccurrences shocked neurosurgeons but they
were reluctant that the growth did stop eventually.

Discussion:
The results showed that in many cases, the tumor will reoccur, but after it reoccurs, the
level of danger decreased and they were able to live with the tumor. Although the hypothesis was
disproved, researchers are still working on finding a cause and permanent treatment to the tumor.
Unfortunately, there isnt enough information on EVNs to give a final conclusion along with
how to create it.

Conclusion:
Extraventricular neurocytomas are difficult tumors that are rare, and not enough is known
about them. To this day, treatment consists of brain surgery and sometimes radiation along with
close monitoring. The success rates varies, but not many people die from the tumors. These

tumors only have been know for about 20 years, and there is still currently a lot to learn about
them.
