Artifact Title: Spring Conference Form

Description of artifact and how the artifact meets the standard:
This form is used at the spring conferences. Unlike the fall conferences, these are student led. Again we ask
parents to bring their child to the conference. The students complete this form the week prior to conferences. I
walk them through the form and discuss what each component means. I also tell students that I will be
looking through the forms after they are turned in. If I feel a student has not accurately communicated how
they are doing, I add my own notes to it. I remind the students that it is better for parents to hear the truth
from them. At conferences the students share the form and lead the discussion on why they have selected each
answer or explanation. As we move through the form, I add in comments and feedback as to why I agree or
disagree with the student. If a more thorough conversation needs to happen we use this time to do that.

Reflection on artifact:
Many students are nervous to be the center of attention and being the primary speaker. In an attempt to
relieve students of some of this stress, I plan to do a practice conference with each student. I will show them
what Ill be asking them to do and they can practice speaking. If they have questions we can talk about them at
that time. It will also provide one last opportunity for students to answer honestly on their form. We can have
the discussion about why they have scored themselves the way they did, and whether or not I agree.
Typically these conferences go well and students realize they had nothing to be stressed about. Parents enjoy
seeing their childs growth since fall conferences, and also typically are surprised at the letters their child has
written to them. The students write wonderful letters and the experience is most often very successful.
