1. COMPETENT AND SAFE PRACTICE WITHIN A PROFESSIONAL, LEGAL, AND ETHICAL

1. 1 Comply with regulatory standards, relevant legislation and the practice settings policies and

D.A.S. : I believe that i do comply with regulatory standards , an example would be when there has to be a registered nurse in the room for a student
nurse to be feeding a resident. That is also sunny brook veterans policy. As well as having a registered nurse in the room when we are using the
hoyer mechanical lift for the resident, it always has to be a registered nurse using the hoyer lift for transferring the patient from bed to chair.

1. 2 Act in a responsible, ethical and accountable manner.

D.A.S. : I do this by holding myself accountable for my actions in the clinical setting. Nothing has gone wrong, but obviously if something ever did, it
would be of my professional behaviour to hold myself accountable for such actions.

1. 3 Determine strengths and limitations and when to consult and seek assistance from other

D.A.S. : I believe that i already do this, in the beginning of the semester i did not really have any strengths but now i feel confident with handling two
patients, and feel really confident with providing the AM care, and administering medications. I now only really seek assistance from other
professionals when i need help with the transferring, or if i am really unsure on how to do something.

1. 4 Use evidence informed practice and a theory-based approach to care.

D.A.S. : I believe i have started to used more of my theory-based approach to care from my theory classes. I would not perform an assessment on an
patient unless i knew the reasons for doing such an assessment.

D.A.S. : I always ensure client safety and a safe environment by always making sure that the bed rails are put back up after i am done at the bed
side. I make sure to always ensure safe feeding practice. Encourage swallowing of food, eating, and drinking fluids. I always make sure that the
patients seat belt is on when in the chair and that their bed alarm is attached.

1. 6 Ask relevant questions to clarify understanding of the plan of care and whenever uncertain.

D.A.S. : I have no problem doing this, I may ask Neemara or ask the primary nurse for that specific patient. I always look at the patient's care plan, so
it provides an accurate image in my head of what to do for the patient. I always ask the nurses when i am uncertain, or look at the care plan of the
patient.

1. 7 Identify ethical and unsafe situations and seek appropriate assistance to resolve these.

D.A.S. : I believe i do this , during clinical on the second last week, i walked by a patients room and noticed that she was sleeping without her bed
rails up. I immediately went and put the bed rails up in the room because she was on the edge, and than i asked Neemara about it, and we asked one
of the primary nurses and she said that the patient puts her self to sleep and only needs 1 side rail up. It explained a lot because i thought that a nurse
put the patient to bed and forgot about the side rails.

1. 8 Demonstrate understanding about the role of the student nurse.

D.A.S. : I do understand my role of the student nurse, when i first started at sunny brook veterans i was unsure, but it has been a great learning
experience and every week i have attended clinical i have got stronger in my nursing student skills, and more confident with providing care to
residents and administering medications.

2. ACCURATE AND RELEVANT COMMUNICATION


2. 1 Participate in team sharing and discussions.

D.A.S. : As a nursing student team we all participate in sharing our experiences and discussions during breaks, and lunch, and also when we have
post conferences and Neemara asks us all individually how our day was.

2. 2 Document clearly, accurately, concisely in a timely manner using written and electronic methods.

D.A.S. : Yes i do this by using the POC at sunny brook veterans. They actually made it very easy to document, and i always document everything i
do for a patient after i provide the care.

D.A.S. : If i notice some abnormal behaviour in a resident that i am taking care of than i will report it to the primary nurse and they will tell me if we
should write a progress note or not. Typically they will write a progress note about a patient who has abnormal behaviour or does something
abnormal.

2. 4 Utilize communication techniques with the client and inter-professional health care team.

D.A.S. : I use several communication techniques when working with the patients of K3E because they all have dementia. I find that it is good to talk
to them on their level and not over them, and to always agree with whatever they see even though they may be defiant at times. Also, talking in front
of them to their face instead of directly above them or behind them. I also, relay all this information to the inter professional health care team.

2. 5 Maintain privacy and confidentiality according to the Personal Health Information Protection Act
(PHIPA)

D.A.S. : I always maintain privacy and confidentiality of every patient and sunny brook veterans.

2. 6 Use technology to retrieve information including research, data and other information.

D.A.S. : I use my online textbooks when i need to find information about a specific drug that i am administering to a resident. Also, i retrieve
information from the college of nurses.

3. CARING RELATIONAL PRACTICE

D.A.S. : Yes, i believe that i have utilize effective communication techniques, i always relay information to the nurses and i know how to communicate
with various types of residents. I never knew how to communicate with an individual who was blind, but now i know how.

D.A.S. : I believe that i also demonstrate a therapeutic caring and compassionate relationships with the clients of sunny brook veterans. I also enjoy
talking to every patient there, they are all so unique and i love hearing about their past life and getting them to reminisce.

D.A.S. : I apply knowledge of therapeutic use of self and identity by getting the client to reminisce about their past life and encouraging to talk to them

D.A.S. : Many different nurses and even student nurses may have different beliefs and values and personal exeperiences that may impact their
nursing practice, but i do not let mine get in the way of the objective of the client. Which is providing a caring approach to their care.

D.A.S. : The best part about clinical is being able to interact with each client, and helping the nurses of the inter professional health care team. I have
learned so much about each client and about some of the nurses in K3E.

D.A.S. : I believe that i have done this because i have had a few female clients that do not like male nurses caring for them, so i had to let my female
colleagues care for them. Also, one of the patients at K3E have religious beliefs that basically mean she can't receive any medications from them
receive transfused blood and does not want male nurses caring for the client.

D.A.S. : I always provide effective client education by encouraging the client and explaining to them about the care that i am providing if they ask
questions about it. Sometimes they ask why do i need a shower and i educate them on what a shower does for their body.

D.A.S. : I think that i have worked on this since the midterm evaluation. When i recognize conflicts, i resolve it in a polite and calm manner.

D.A.S. : I believe that i have demonstrated leadership skills that foster group work by helping one another, and i believe that it has always been a

D.A.S. : There have been several opportunities to improve health care service by advocating for clients, for example the one nurse that was last on
shift for a patiient forgot to order more oral suspension medication for a client , so myself and my clinical instructor filled out the pharmacy form to get
more for the patient.

D.A.S. : My nursing student team and I always provide feedback to each other on our nursing practices. It actually helps a lot when a colleague
notices something that we can improve on.

D.A.S. : The one time i had a patient refuse their medication, so i told the primary nurse and my clinical instructor and i supported the clients rights for
refusing the medication and their own personal choice

D.A.S. : I respond appropriately to unsafe and unprofessional behaviours but i also act in a professional behaviour while i attend clinical.

D.A.S. : I do this by always washing my hands before providing care, and after providing the care, this carrying out an effective nursing practice in
best interests of the public by infection control, and if a client is in isolation, i always wear the proper material for precaution.

D.A.S. : I always do this by looking at the care plan of the specific patient and use that for their care.

D.A.S. : Yes i have done this by telling the primary nurse that they had the wrong information on the care plan for something to use, so than we fixed
it.

D.A.S. : At first when we started i was not doing my nursing care in a organized and timely manner, but as the weeks progress, i feel more confident
in completing my nursing care in a responsible time and organized manner. For example, by obtaining all the supplies i need prior to going into the
room and starting my care.

D.A.S. : I have continued to do this if i do not feel comfortable in an area that i am caring for the patient in. But other than that, i know when to seek
assistance if i am going to have trouble. It is better to have assistance than to be struggling by myself.

D.A.S. : We continue to use team work among ourselves as nursing students. My colleagues have had some troubles with their patients, and if i am
not busy caring for my patient at that time when they are having trouble, than i have no problem helping them with their patients if they need the help

D.A.S. : I feel like i self reflect every week after clinical on thursdays. I find that my strengths are through communication and starting to get the
handle of the flow of AM care, but i find my weaknesses are still getting the patients dressed on my own. For future weeks, my goal is to do the
dressing of a patient all on my own.

D.A.S. : I find that this is what clinical is all about . The learning opportunities. So far, i have learned a lot from just attending clinical. For example, I
learned how to bathe a patient, and how to administer medications. Administering medications is my favourite part and makes me feel more
professional as a student nurse.

D.A.S. : I have been doing this by providing the knowledge that i have obtained from the nursing theory class to my nursing practice, which it actually
helps a lot.

D.A.S. : At the start of the semester i did not really know how to prioritize my time and organize my utilities that i would use while providing care. Now
i feel more confident in prioritizing my time and organizing my care for that patient.

D.A.S. : I am always maintaining a professional attitude towards my patients and other members of the inter professional health care team. I will
always be accountable for what do. I need to also advocate for a patient when necessary as well.

D.A.S. : I recognize my strengths, and i believe that i have got a lot more stronger than when we first started in the clinical setting. I am now confident
with the AM care that includes bathing the patient, dressing, oral care, transferring, changing briefs, feeding, and administering medications. I always
seek assistance and support when i am unsure about something or if i do not know how to provide the care.

Student Final Summary with Strategies for Continued Growth


Overall, i have learned a lot from my clinical experience at sunny brook veterans. It has been a great learning experience and a great environment for
first nursing student to patient contact. I believe that the knowledge and skills i have gained from attending clinical at sunny brook veterans will stay
with me for my future nursing practices. My final learning strategies would be to still learn how to prioritize my time in a more efficient way and
organize my tools better. But as each week went by in clinical i felt like my skills have got a lot more stronger, and i feel more confident in providing
care to patients as a student nurse. Every nurse at sunny brook veterans has shown me a lot on how to care for patients, i am very thankful for this
experience. It has been a fulfilling an rewarding experience attending clinical at sunny brook veterans.
