INFORMATION FOR: COUNT 1: CHILD ABUSE BY INJURY ~ 21 0.S. § 843.5(A), a FELONY COUNT 2: CHILD NEGLECT ~ 21 0.5. § 843.5(C), a FELONY ‘STATE OF OKLAHOMA, COUNTY OF MUSKOGEE: |, Onvil Loge, the undersigned District Attorney of said County, in the name and by the authority of the State of Oklahoma, give information that in said County of Muskogee and in the State of Oklahoma, RACHEL EAN STEVENS did then and there unlawfully, willfully, knowingly and wrongfully commit the crime(s) of: COUNT 4: CHILD ABUSE BY INJURY ~ a FELONY, on or about the 8th day of December, 2015, by wilfully/maliciously abusing, L.G., who was 5 years old at the time, resulting in broken bones, bleeding of the brain, lesions on his head and neck, various scabs and bruises about his body, all occurred while in the custody and care of Rachel Stevens and Kayla Jones. COUNT 2: CHILD NEGLECT ~ a FELONY, prior to the 8th day of December, 2015, for a period of time of approximately six months, by wilfully/maliciously neglecting, L.G., who was 5 years old at that time, by failing to get treatment for seizures and broken bones, proper medical attention, and malnutrition, all occurred while in the custody and care of Rachel Stevens and Kayla Jones. THE STATE OF OKLAHOMA, ( Plaintiff, “vs. Rachel Stevens Kayla Jones Defendant(s), Affidavit ‘The undersigned upon oath deposes and states as follows, to wit: Affiant advises that on__12/08/2015 at approximately 2100 _hours that the following i occurred at 2518 E. Harris Ro: _» Muskogee, Muskogee County, Oklahoma: The victim was taken to St. Johns Hospital in Tulsa for his injuries. ‘The victim was then air lifted to the OU Medical center in Oklahoma City. The victim was taken to St. Johns due to the lesi After being at the OU Medical Center it was identified thatthe victim had multiple broken bones i stages of healing. It was also identified that the vic m had malnutrition. Later x-rays would show th e first ical Center| he suffered 2 strokes due to een tied up, duct tape over his eyes and ha been locked in a room. The victim also disclosed that both suspects had hit him on the hand, head and butt with a belt, also disclosed that the mother hit the victim on the hand with a hammer. ‘The victim disclosed that the stepmother kicked him in the groin so hard that made him bleed. Through interviewing the mother of the victim it was determined that he has had two seizures and was not taken to the doctor or hospital right away. The mother of the victim admitted that she should have taken him to the doctor or hospital to get the lesions looked at and to be evaluated for the seizures. Rachel Stevens is the victims mother and Kayla Jones isthe stepmother of the vietim. ~ Based on this information, the undersigned prays that this Honorable Court issue a finding OF cause exists to believe that a crime has been committed and that there is probab| defendant(s) above named committed that crime.
