AFFIDAVIT OF SUPPORT
I, ARMANDO DE JESUS, of legal age, Filipino Citizen,

after having been duly sworn to in accordance with law, do
hereby depose and state:

That Lorraine Villanueva , is my daughter of legal age,

Filipino and with the same address at 41 Augusta St. AyalaHill

Estates Tandang Sora Balara Quezon City as my dependent;
2.

That I am the one providing financial and material

support to my daughter, the afore-named person, due to her

condition and that she is dependent upon me for financial and
material support;
3.

That I execute this Affidavit to attest to the truth of the

foregoing facts and for whatever legal intents and purposes this
may serve.
