Housing Reservation Form

FORM MUST BE TYPED!!! Upload form to https://georgiafbla.wufoo.com/forms/slc-housing-upload/ from January 25-31.

Must be received by January 31, 2016. You must make this reservation through the Georgia FBLA Hotel Reservations Desk to stay
at the hotels. Assignments will be made on a first-come, first-served basis. Members must be registered for the conference
before hotel assignments will be made. If you are not registered for the conference, assignment will not be made until your
members have been registered for the conference. No phone reservations accepted.
You must mail deposit upon reservation confirmation (check made payable to hotel) or include a credit card number before room can
be booked (credit card will not be charged)
Reservations may not be made prior to January 25. All cancellations must be made prior to February 15; failure to do so may result
in forfeiture of deposit.
If you qualify for tax exemption, you must submit a completed copy of your tax exempt forms directly to the hotel upon assignment.
Tax exempt forms must be sent to hotel prior to arrival.
If you must to be placed at the same property with another school, forms must be submitted at the same time on the same
upload form. If they are not submitted on the same upload form, we cannot guarantee that you will be placed at the same property.
