Atelektasis
Definisi :
Suatu kelainan parenkim paru yang
mengalami pengembangan paru-paru
yang tidak sempurna, ditandai oleh
alveolus pada bagian paru-paru yang
terkena tidak mengandung udara atau
kolaps.
Bisa mengenai seluruh paru-paru atau
sebagian kecil saja.
Etiologi
Obstruksi
Penekanan ekstra pulmonal
Pneumothorax, cairan pleura, peninggian
diafragma, herniasi organ abdomen ke
dalam rongga thorax, tumor mediastinum
Paralisis atau paresis gerak pernafasan
Hambatan gerak pernafasan akibat
kelainan pleura atau trauma thoraks.
Mekanisme Atelektasis
Atelektasis resorbsi
Patofisiologi :
Adanya penyumbatan saluran udara
menyebabkan udara tidak masuk ke
alveolus sehingga alveolus kolaps.
Tekanan ekstrinsik menyebabkan
udara terdorong keluar sehingga
paru-paru akan kolaps.
Radiologis :
Tanda langsung
Perubahan letak fissure interlobaris
Penambahan opasitas
Perubahan corak bronkhovaskuler crowded
Adanya pengurangan volume bagian paru
baik lobaris, segmental atau seluruh paru
sehingga aerasi paru berkurang
Macam Atelektasis
Atelektasis lobaris
bawah,
Komplit kolaps dari
lobus bagian kanan
bawah dengan
kehilangan volume,
ditandai oleh
pergeseran trakea
dan batas jantung
ke bagian kanan.
Udara di bagian
kanan bawah di
absorpsi,
menghasilkan
diffuse infiltrate

Lateral view:
RLL Atelectasis:
Triangular opacity

lateral border is the
major fissure (not

frontal view). Right
hilum is displaced
caudally and
partially obscured.
The hyperexpanded
RML outlines the
cardiac border and
right
hemidiaphragm.
LLL Atelectasis:
Notice the wedge
shaped opacity
behind the cardiac
silhouette. The
border is formed by
the major fissure
(arrow). The left
hilum is partially
obscured and
displaced caudally.
The left upper lobe
is hyperexpanded
accounting for the
increased lucency
in the left
hemithorax.
Post-obstructive
atelectasis of
RLL: The major
fissure is visible
as it has rotated
into view. There
are no air
bronchograms
seen within the
atelectatic region
of lung. The
patient is
intubated. The
obstruction is
likely due to
mucous
plugging.
