Sample Grading Rubric for Clinical Competencies
Giving a subcutaneous insulin injection
(Student must receive 1 point in each box = 11 points
Student finds BG level, and calculates the amount of insulin
required for injection
Wash hands, assemble equipment
Remove the cap on the medication vial, clean the exposed rubber
stopper with alcohol wipe.
Remove the syringe from the sterile package and attach the needle
securely.
With the needle capped, pull back the plunger, filling the syringe
with air equal to the amount of medication to be administered.
Inject the air in the syringe into the vial. With the needle
completely covered by the medication fill the syringe with the
correct dose of medication. Tap the syringe to be sure that no
bubbles are present. Be sure the correct dose is drawn in syringe.
Perform safety checks with medication (right drug, right dose,
right time, expiration)
Carefully recap the syringe using the scoop method with no
contamination. Student must restart if any contamination is
present.
Place gloves and Chose appropriate site for medication, and clean
with alcohol swab in circular motions from outward to inward
motion.
Grasp he patients skin between the thumb and index finger, insert
needle at 90 degrees and slowly give injection.
Remove needle and carefully engage needle guard. Discard in
sharps container
