a. Analyzes and applies data from multiple assessments and measures to

diagnose students learning needs, informs instruction based on those
needs, and drives the learning process

The above pictures represent the process that was taken based on formative
assessment results and what was done to best support our students learning and
need for reteach. The Mathematical Formative II assessment results were analyzed
to reveal where the class was as a whole where they could use additional support.
This information was then used to create morning bellwork problems for students to
practice and an opportunity for us teachers to reteach. I have also used exit tickets,
independent work, and quizzes to inform where my students are at with mastering
an objective. This information was then used to create small groups where students
who have mastered the content can lead their peers during a reteach. In addition, I
have also used the results to create my reteach and enrichment groups.

I believe it is important to give students multiple opportunities to demonstrate their

understanding of a concept so that I can redirect their misconceptions or
misunderstandings of an objective so that they can master the content. In addition
to the typical independent work and homework as a means of practicing a new skill,
I also like to provide opportunities for collaborative work, whether it be through
center activities or paired task work. In addition to those opportunities for students
to demonstrate what they know, I will also use some of the chapter checkpoints to
track their progress. By offering students a variety of different modalities to show
their understanding, I feel that I am able to best identify which students have
mastered the content and which students could use some additional support.

achievement and learning gains.

In order to assess my students, I try to do so in a variety of ways, both informally

and formally. I have used checklists to help notate whether a student has mastered
the learning objective through observations in the classroom. I have also used exit
tickets to see if students have mastered the learning objective as well. As a more
formal way of assessing the students, I use the mid-chapter checkpoints in Math to
monitor the progress that students have made and what may need to be reviewed
prior to the final unit assessment. Looking at all of these pieces of data allows me
to see where the students began and where they ended in order to see their
learning gains as a whole picture.
d. Modifies assessments and testing conditions to accommodate learning
styles and varying levels of knowledge.

I know that all of my students are unique and they all come with their own learning
styles. For this reason, I believe it is important to also keep that in mind when
testing students because not all students test the same. Some students may like to
be in the front of the room, while others may want to be in the back of the room
during testing. One modification that we make for a student in our class is the use
of the privacy partition, regardless of the seating arrangement in the classroom.
e. Shares the importance and outcomes of student assessment data with
the student and the students parent/caregiver(s)

After we assess the students at the end of a unit, we will have the students take the
test home and have the parent/caregiver sign the test, indicating that they saw it.
Another way that we have communicated assessment results to students and
parent/caregiver(s) was during our conference night. We used this time as an

opportunity to really individualize the assessment based on each student and where
some of their strengths were as well as what they needed some additional support
with. Suggestions were offered to the parents and some additional work was
provided to students and parents for the extra practice if they expressed interest.

f. Applies technology to organize and integrate assessment information.

I believe it is important to maintain student grades in an efficient, organized manner

that one can easily access at any given time. My experience has been with Edsby
and creating various different tasks that count towards the overall class grade. I
have found that it is important to grade and account for the grades in a timely
manner, otherwise it is easy to fall behind.
