AFFIDAVIT OF LOSS

I, MARY KAREEN CHESTER L. ABAS, of legal age, Filipino, single and a resident
of Sanciangko St., Cebu City, after having been duly sworn to in accordance with law, depose
and state THAT :
1. Last March 23, 2016, I visited Tabor Hills for the purpose of doing Station of the Cross
in adherence to our religious practice during Holy Week;
2. Because it was quite awkward for me to wear my school I.D at the vicinity of the
Tabor Hills, I placed my School I.D bearing number 13100432 at the back pocket of my jeans;
3. It was when I was about to leave Tabor Hills when I noticed that my School I.D was
lost;
4. Efforts were exerted to locate the same, but in spite of diligent search, it could not be
found creating a belief that I probably inadvertently lost it somewhere which is now beyond
recovery;
5. As such, I am executing this Affidavit of Loss to attest to the truth of the foregoing and
for the purpose of issuance of a new School I.D.
