Hazard
Cement bricks were scattered on the pathway for
workers and the workers were using this way.
Consequence
Scattered cement bricks on pathway can cause
tripping and falling off workers and thereby serious
injuries can happen to them.

Excavation was going on at site and the reposed
soil was kept near the edges of excavation. Safe
distance from the edges of excavation for keeping
reposed soil was not maintained.
Consequence
Reposed soil may fall down to excavation and
thereby serious injuries or even death of workers
can happen.
Hazard
Underground water pipe line found inside the
excavation. Pipe line was slightly broken and water
was coming out of the hole on pipe line. Water line
was not isolated.
Consequence
Hole on pipe line can get bigger and thus
excavation may get flooded and thus drowning and
thereby death of workers working inside
excavation can happen.

Hazard
In the canteen for workers, uncovered waste bins
were found. Waste bins were full with food waste and
food waste were scattered around it. Flies were
present over food waste.
Consequence
Flies over food waste can spread diseases like cholera,
dysentery etc to the workers.

Clear the waste in waste bin and the


waste scattered around the waste bin.

Provide sufficient waste bins in canteen


to avoid overflow.

Staircase giving access to first floor was found wet
and slippery and the workers were using this way.

At one part of work site, piling was going on and
heavy noise was coming out of piling equipments.
Consequence
Exposure to heavy noise from piling equipments can
cause NIHL, tinnitus etc. to the workers working
nearby.

Workers were found using welding machine whose
cable insulation was broken at several places.
Consequence
Usage of cables for welding machine with broken
insulation can lead to severe electric shock to the
welders as well as workers passing nearby and
thereby serious burns and death can happen to
workers.
Hazard
Mobile crane was operating at work site. Safe load
was not maintained on it. SWL certificate was absent
for the mobile crane. Workers were found working
beneath the suspended load.
Consequence
As safe working load was not maintained on mobile
crane and as SWL certificate was absent on it, the
load may fell on top of workers working beneath and
thus serious injuries or death can happen to the
workers.
Hazard
Jack hammer was used by the workers for concrete
breaking near entrance of work site without wearing
PPEs against vibration from the equipment. The
process was continuous and exposure to vibration
was maximum.
Consequence
Exposure to heavy vibration for a longer duration can
cause HAVS, WRULD etc. to the workers.

Substitute the equipments with those of Immediate


lesser noise or provide appropriate
PPEs such as ear defender, ear muffler
etc. to the workers.

Provide proper training and awareness
to all workers.
Stop the work and the welding

Welders were found using welding glass instead of
face shield during welding.

Workers were found using grinding machine which
was without guard.

Consequence

reversing can cause collisions of it with the workers
and thus serious injuries or even death of workers
can happen.

Workers involved in steel frame work erection were
exposed to direct heat from sun for a long duration.
Consequence
Health problems like dehydration, heat cramp, heat
stroke etc. can happen to the workers due to direct
heat exposure for a longer duration.

Workers involved in concrete mixing were not worn
nose mask and were exposed to cement dust.
Consequence
Cement dust can be inhaled by the workers and thus
breathing problems, lung cancer etc. can happen to
them.

Hazard
On the first floor of the building, welding work was
going on. Wastes like papers, empty cement bags etc.
were scattered around the welding area.
Consequence
Flying hot particles coming out of welding process
can give ignition to papers, empty cement bags etc.
and thus fire can happen and due to the spread of
fire, severe burns to the workers in the proximity and
damages to the material can happen.

INTRODUCTION INCLUDING OVERVIEW OF THE AREA INSPECTED AND ACTIVITIES TAKING PLACE

Site visit was carried out on 26 Sep 2014 at one of the construction site of Shobha
Rustomjee

Ind. Pvt. Ltd. is one of the leading construction companies in
Pvt. Ltd., Thane. Shobha
Rustomjee
Constructions
India. The major activities of the company are to construct housing buildings, complexes, roadways
etc. The area of construction site was approximately 8 acres and there were approximately 300
workers working in each shift. Office of the construction site was situated near the entrance.
Construction related activities like concreting, cement plastering, loading and unloading, mobile
crane operation, fork lift truck operation, hot works like welding and cutting, steel frame work
erection etc were going on during the visit. The purpose of the visit was to identify the health and
safety standard maintenance of the company.

EXECUTIVE SUMMARY
This report focuses on five major issues which were identified at work site and were
immediately dangerous to health and life of workers. Elimination of these issues is required to avoid
legal, social and economical implications on the company. These issues are listed below as per the
order of importance.
Risk of falling of objects on top of workers
Reposed soil was kept near the edges of excavation without maintaining safe distance from
excavating area. Falling of reposed soil on top of workers working inside excavation can lead to
serious injuries or even death. This issue to be addressed immediately.
Risk of drowning of workers
Underground water pipe line found inside the excavation and it was leaking too. Water supply was
not isolated before commencement of excavation. Leaked water can lead to flooding of excavation
and in turn death of workers due to drowning. This issue requires immediate remedial action.
Risk of falling of workers from height
Non compliance of PPEs like safety harness while working at height can lead to falling of workers
from height and thereby serious injuries or death can happen to them. At site, workers involved in
steel frame work on second floor were without safety harness. Issue is serious and to be addressed
immediately.
Risk of explosion
Welding was going on and nearby it was found that acetylene gas cylinder is leaking. Acetylene gas
can catch fire from the flying hot particles coming out of welding process and thereby heavy
explosion can take place and in turn death of workers in the vicinity. Issue to be seriously considered
by the management.

Risk of electrocution to workers


Welding equipment with broken cable insulation was used at site. High voltage current coming out
of broken cable can cause severe electric shock to the workers and thus severe burns or death of
workers can happen. Issue to be addressed immediately.
Very serious consideration to these issues can reduce accidents and thereby costs to be afforded by
the company following an accident. Legal implications also can be avoided.

MAIN FINDINGS OF THE INSPECTION

Risk of falling of objects on top of workers (Observation No. 03)


Excavation was going on at site and the reposed soil was kept near the edges of excavation. Safe
distance from the edges of excavation for keeping reposed soil was not maintained. Reposed soil
may fall down to excavation and thereby serious injuries or even death of workers can happen.
Recommended Control Measures

In short term, remove the reposed soil from near the edges of excavation.
In long term, depute a competent person to monitor the activity.

Cost to the management


The estimated cost for these measures is about Rs. 12,000/Breaches

Risk of drowning of workers (Observation No. 04)


Underground water pipe line found inside the excavation. Pipe line was slightly broken and water
was coming out of the hole on pipe line. Water line was not isolated. Hole on pipe line can get bigger
and thus excavation may get flooded and thus drowning and thereby death of workers working
inside excavation can happen.
Recommended Control Measures

In short term, stop the excavation activity and isolate the water supply to the pipe line
running beneath excavation.
In long term, water pipe line to be re routed away from excavation area.

Cost to the management


The estimated cost for these measures is about Rs. 30,000/Breaches
Pipelines Act 1967, Occupational Health and Safety Act 1985

Risk of falling of workers from height (Observation No. 08)


Workers found doing the steel frame work of second floor top and they had not worn safety
harness. Non usage of safety harness by the workers while working at height can lead to falling of
them from height and thus severe injuries like head injury or even death can happen to them.
Recommended Control Measures

In short term, enforce the workers to wear safety harness while working at height and if
insufficient, and provide them with the same.
In long term, training programme to be conducted for workers working at height regarding
the safe practices to be followed while working at height.

Cost to the management


The estimated cost for these measures is about Rs. 30,000/Breaches

Risk of explosion (Observation No. 10)
Acetylene gas cylinder was found leaking and welding was going on nearby. Leaked acetylene gas
may catch fire from flying objects coming out of welding process and the fire in turn can cause the
explosion of acetylene gas cylinder. Explosion of cylinder can cause severe injuries or even death of
workers in the vicinity.
Recommended Control Measures

In short term, stop the welding work and stop the leakage of acetylene cylinder.
In long term, replace the cylinder with a new one.
In long term, awareness programme on safe practices to be followed during hot work to be
imparted to welders.

Cost to the management


The estimated cost for these measures is about Rs. 20,000/Breaches
Explosives Act 1884, Gas Cylinder Rules 2004

Risk of electrocution to workers (Observation No. 12)


Workers were found using welding machine whose cable insulation was broken at several places.
Usage of cables for welding machine with broken insulation can lead to severe electric shock to the
welders as well as workers passing nearby and thereby serious burns and death can happen to
workers.
Recommended Control Measures

In short term, stop the work and the welding equipments cable to be insulated temporarily
with insulating tape.
In long term, renew the cable of welding equipment.

Cost to the management


The estimated cost for these measures is about Rs. 5,000/Breaches
Indian Electricity Act 1956, Indian Electricity Rules 1956
Conclusion
A lot of good practices were followed at the work site. Maximum of the workers were adhered with
safety rules at site. Compliance to PPE was up to the standard. Management should encourage the
workers to continue with the good practices followed at site to enhance the health and safety
culture of the company. Serious issues found at work site have been mentioned on this report with
consequences. Appropriate control measures have also been suggested to eliminate these issues.
Thanking the management of Rustomjee Constructions Pvt. Ltd. for allowing me to visit and inspect
their site. At the same time requesting the management to discuss the serious issues mentioned in
the report in forthcoming safety committee meetings to avoid social, legal and economical
implication on the organization.
