QA Program Learning Plan form

Collection of Personal Information


The College of Nurses of Ontario (the College) collects the information in the QA Program Learning Plan Form if you have been selected for Practice Assessment
under the general authority of the Regulated Health Professions Act, 1991, S.O. 1991, c. 18, the Nursing Act, 1991, S.O. 1991, c. 32, and its regulations; and the
Colleges by-laws. The College collects the information for the purpose of assessing your continuing competence through its Quality Assurance Program. Appropriate
measures are taken to safeguard the confidentiality of the personal information you provide and all documents become the property of the College.

1) Consult with nurses and clients' caregivers to learn more about client needs.
2) Determine ways to effectively communicate with different clients.
3) Take action in performing and demonstrating new skills in the clinical setting.

1) Review health assessment checklists.


2) Schedule when to do assessments according to client's daily schedule.
3) Ensure documentation is completed in a timely manner.
4) Complete care plan before the end of the shift and review with teacher.

My learning goals focus on providing client-centred care. Becoming a more


effective leader and health care provider ensures my clients that I am
capable and competent enough to care for them. Improving on my care plans
will enhance my skills of prioritizing interventions.

What did I learn?
What impact has my Learning Plan had on my practice?

I felt that enhancing my leadership skills required more planning than what I
first planned. I learned the becoming a better as a student nurse is more than
just being able to perform procedures confidently in the clinical setting. I
realized that involves a lot of times management between multiple clients,
documentation, breaks, and collaboration with peers as well as the
inter-professional health care team. I feel that in my next placement I will be
able to improve more on my leadership skills through communication and
collaboration with the inter-professional healthcare team.
Creating care plans for individual clients are very detailed. I learned that I
should focus on assessing the whole individual through the specific systems
such as the respiratory, cardiovascular, neurological, and psychological
systems.
