in my Certificate of Live Birth duly issued by the National
Statistics Office is MARIA DINELLAH CABRIA;
2. Since childhood, however, I have been erroneously using
MA. DINELLAH CABRIA as my first name, instead of the
correct one, MARIA DINELLAH CABRIA;
3. Thus, in all my official records, it is appears that my first
name is MA. DINELLAH;
4. MARIA DINELLAH CABRIA as appearing in the Certificate of
Live Birth and MA. DINELLAH CABRIA as appearing in all
official records, therefore, pertain to one and the same
person;
5. I am executing this affidavit to attest to the truth of the
