H.M.T. : I learned from theory is that dementia patients often get confused and agitated easily, one of suggested way to communicate with
them is to face the patient, not shouting but using a firm voice with respect and giving them time to respond (Potter, Perry, Ross-Kerr,

H.M.T. : I am aware of safety practices when giving care for patients. For example, before leaving the patient's room, I have to make sure
that alarm bed is on, patient's side rails are up and bed is at low position as well as patients understand and are able to reach the call bell if
they need assistance.

H.M.T. : I agree that clarification is very important. For example, giving fleet enema as prescribed, if the order says that giving fleet enema
if no bowels movement more than 3 days. I need to clarify with the nurse, consult her opinion if we need to give fleet enema as scheduled
since the progress notes show patient got normal bowel movement yesterday. By doing it, I can avoid more harm to the patients because
every medication has adverse effects, including electrolyte imbalance if patients has too much loose stool and dehydrated.

disease pathology, the medication uses and other considerations.

follow up the care for specific patient.

patient, so that I can report to her about his condition or look for additional advice during my care.

H.M.T. : The majority population of patients are old, their skin is often dry and sensitive. When giving bed bath, I am aware by letting them

collaborate with each other in providing bed bath since he is very heavy and changing his brief properly before the nurse provides
mechanical lift to move the patient to his wheelchair.

H.M.T. : I am aware of patient's choices and preferences by learning from their care plan carefully. For example, one of my patient likes to
have quiet environment, but he likes scenery; so I take him to the lounge and let him sit next to the window where he can enjoy viewing
more trees and activities outside.

H.M.T. : One of my patient is diagnosed with COPD (chronic obstructive pulmonary disease), so I encourage him to practice deep
breathing and purse lip to help him slow down the breathing pattern and improve the lung capacity to exchange gas.

H.M.T. : For example, patient refused to drink water instead of drinking cokes. I find that there is a conflict between client well-being and
client choice (College of Nurses of Ontario, 2009). As a student nurse, I should promote the best interest for her well-being by encouraging
her to drink water instead of coke since she has diagnosis of diabetes and hypertension, however, at the same time I need to respect her
choice of choosing the liquid to drink. In response to that, I don't stop her drinking coke but repetitively try to encourage her to drink,
saying the benefits of water and asking for additional advice from staff to motivate her drinking water.

H.M.T. : If there is a time conflict, though we all agreed from beginning to help each other in providing care for both patients. After
providing bed bath for my colleague's patient, my colleague realized that she needs to feed her patient and was not able to work with me
for my patient's bed bath. I understand her time conflict and mine, I talked to my clinical teacher and asked for other colleague for
assistance. By doing that, we all can provide the safe care in timely manner.

H.M.T. : I believe one of the best way to improve learning need is to obtain peer feedback. I believe that being open to peer's suggestions

H.M.T. : As a nurse, we need to adhere to practice standard and policy provided by college of nurse of Ontario, according to practice
standard for ethics, we need to maintain my commitment to clients such as respect the clients, to self such as recognize what I truly
believe in for example, I always value and respect the elders which will have more influence on the way caring for them. Also, ensuring the
commitment to my college, for example supporting and collaborating in providing care for patients, and the commitment to quality practice
environment such as documenting after providing care and following the practice setting policy (College of Nurse of Ontario, 2009)

H.M.T. : I follow the practice standard by being aware of client's right for making choice. After providing patients with benefits for
increasing more fluid intake, if patient still refuses to drink. I have to respect the patient's choice decision (College of Nurse of Ontario,
2009); then report for the total intake so that the health care team can follow up with the patient's fluid intake and able to provide fluid
supplement such as hypodermoclysis if his fluid intake is less than requirement.

H.M.T. : Creation is very important, especially giving care for dementia patients with behavioral changes. Patient may response different
today comparing from yesterday; thus, being creative, be patient and firm in providing care is encouraged.

assessment based on his conditions (able to provide part of his care, or total dependent on staff) and integrate his preference and his/her
belief and value into his care plan.

H.M.T. : A good theory based plan of care should be client centered, holistic, accessible and should be regularly modified to meet the
client's needs based on his physical and mental changes.

H.M.T. : All nurses should be aware of levels of priorities: first level issues must be prioritized includes ABCs plus V (airway, breathing,
circulation, and vital sign concerns like high fever), then second level issue such as pain, mental changes and then third level such as
providing rest or client education (Jarvis, Browne, MacDonald-Jenkins, Luctkar-Flude, 2014).

caregiver, nurse, doctor, patient's family members and others such as speech therapist, music therapist or physiotherapist.

H.M.T. : Before providing any care, I need to understand the patient's care plan for ensuring patient's safety; or make sure I understand the
procedure of provision care. For example, in a situation if patient has catheter bag need to be changed, I need to review theory for proper
procedure of changing the catheter bag and make judgement about the uterine volume and quality so that I can ensure if patient has a risk
of infection or dehydration and others.

H.M.T. : For every time at the clinic, I try my best to contribute to provide the best care for my patient, ensure their safety and collaborate

H.M.T. : My intention for caring is always respect the client, inform their right to choose the care or refuse it. Talking to them to address
their concerns for example, the reason why they don't want to eat, or take medication. Explain to them about how the medication will help
them to relieve pain (for example). If they still don't want, I should consult with other experienced nurses about caring for this particular
patient, so that I can provide the best care for this patient.

H.M.T. : When collaborating with my colleague for giving her patient's care, I take part as a supporter, even though I have taken care of her
patient before. The reason to step back as a supporter, instead of telling her what to do, is because I believe we can learn and respect each
other roles in caring for particular patient; especially, building trust and bonds among team members; but patient's safety is the most
concern.

H.M.T. : Though reading and understanding the patient's care plan, I believe that communicating and asking for specific concerns from the
nurse helps me to understand the patient more, and have a better and right approach to particular patient.

H.M.T. : Ongoing self reflection provides me an outline of areas for my strengths and improvement. Combining the advice and feedback
from teacher and colleague will help me to address the issue more clearly. For example, one of the advice my colleague give me is to work
more on calculation since she believe that accurate measures from calculation has big influence on client's condition such as IV
calculation or ordered and available dose. Taking her advice, I practiced more on several types of nursing calculation which I find
challenging. Now, I find myself more comfortable with it.

H.M.T. : Developing a learning plan have 3 components: Self-Assessment; Practice Assessment and Peer Assessment. Self reflecting

within a time frame for completion. Then, later evaluate the goal.

H.M.T. : I welcome to receive feedback from my teacher and colleague. Taking this advice into consideration, I can develop my learning
need for better nursing practice.

H.M.T. : Being a nurse, we have to maintain the basis knowledge, applied evidence based practice and doing research for improving
client's care and safe workplace environment, such as several resources which include the nursing guideline and standard from College of
Nurse of Ontario or from Registered Practical Nurses Association of Ontario.

H.M.T. : When caring for the patient with benign prostatic hypertrophy, on catheterization and on medication tamsulosin. In order to
understand the care for patient and the effect of medication, I use knowledge learned from school found in chapter 57 for the pathology,
chapter 48 for medications and types of catheterization (Lewis, 2014).

H.M.T. : My skills of using mechanical lift is enhanced; now I find it more comfortable in collaborating with the nurse in transferring patient

H.M.T. : Applying knowledge about medication, for example patients has taken hypertension medication (beta blocker such as atenolol),
the expected outcomes will decrease the blood pressure. Also, by understanding the onset of this medication is 1 hour, and duration is 24
hour, and adverse effects such as dizziness, headache, fatigue, bronchospasm (Skidmore-Roth, 2015) , I might consider while providing
care such as patient's mobility which might create more fatigue or limit the risks for fall since he might feel drowsiness.

H.M.T. : I develop an awareness for patient's safety. Particularly, I remember to put up the rails and alarm bed on, and call bell in patient's
reach before leaving his/her room, or before taking patient to dining room, I should make sure the safety alarm attached to patient, hip
guard on and the seat belt plugged.

H.M.T. : Before proceeding any procedure, I need to ask myself some questions such as if patient has any change on his/her condition, if I
have any experience on this care procedure and if there is any guide practice or assistance for me to provide the care. By considering
these questions, I might start the care by myself or ask for additional support since I am not confident in doing alone.

What are my strengths at the end of the semester?


The three strength I achieve at the end of this semester at Sunny Brook Hospital are:
1. Time management: I am more aware of the time consumed for each task. And following the daily plan helps me to be more organized on my
provision care to my patients and give me time to have enough break.
2. Accountability: I learn that nurses have to take responsibility for their own actions and provide care in a timely concern in order to promote patients
safety, and maintain patients well being. This will become my intention which is always aware of the risks and benefits of the outcomes before
proceeding any action.
3. Hard working: I devote more time in studying such as medical concepts, pathology, nursing interventions and considerations as well as mechanism
action of common drugs such as for cardiovascular, neural (dementia), gastrointestinal, or urological disease. I believe that understanding these helps
me to provide the best and safest care for my client.
I believe that I should focus in some areas for improvement:
1.I want to be able to provide the care in an appropriate way of communication to dementia patients. According to the pathology of dementia disease,
most of patients partly loss or have difficulty in finding words to communicate what they want; and it leads to their confusion and agitation. I would like
to develop more skills in communicating with them so that I can provide them the best care.
2.I want to improve my skills in giving subcutaneous and intramuscular injections. In the unit I am working now, I find it hard to develop my skills in
giving injections.
3.I want to improve my skill in documenting accurately and in timely manner. I understand that nursing documentation is considered important since
this is the most crucial methods which communicates the whole health care team in providing care, interventions and following up and evaluating the
patients condition.
