MEDICAL CERTIFICATE

TO WHOM IT MAY CONCERN:
This is to certify that, RIZZA ANGELA MANGALLENO, 22 years of
age, single, of Purok 1, Barangay Binitaya, Daraga, Albay, was examined
and treated as our patient and was confined from March 20, 2016 to March
25, 2016 with the following findings and/or diagnosis/diagnoses:
Multiple injuries secondary to blunt head trauma secondary to mauling:
1. Tooth (left upper first pre-molar) avulsion;
2. Cerebral contusion with neurologic deficits;
3. Cranial CT scan showed 30 cc epidural hematoma extending
from the left temporal lobe to the left occipital lobe.
The patient needs complete rest of 2 weeks or more.
Laboratory examinations is done and enclosed herewith.
