ONE AND THE SAME PERSON

Erlag Building, Esteban Street, Legaspi Village, Makati, having been duly sworn in
accordance with law, do hereby depose and state that:
1. I am the Corporate Secretary of FURUKAWA SANGYO KAISHA
PHILIPPINES;
2. I caused the preparation of the 2015 General Information Sheet of the
said Corporation
3. The names MIKAMI TAKAHIRO and MAKAMI TAKAHIRO appearing
respectively in pages three (3) and four (4) of the 2015 General
Information Sheet of the above-mentioned corporation refers to one and
the same person;
4. Regrettably, due to oversight I was not able to notice the typographical
error in said 2015 General Information Sheet
5. I am executing this affidavit for the following purposes:
a)
to attest to the truth of the foregoing;
b) in compliance with the Monitoring Sheet of the Compliance
and Monitoring Department of the Securities and Exchange
Commission; and
c) For whatever legal purpose (s) it may serve.
