Compensation Plan, and the Nu Skin Policies and Procedures, the terms of which are fully incorporated herein by this reference. All signatures to this Agreement must be affixed
personally. If this distributorship is a partnership or company, then a Business Organization Information Sheet must be submitted with this Agreement, and an authorized officer of
the partnership or company must sign this Agreement.
By my signature on this Amended Distributor Agreement, I authorize Nu Skin Enterprises New Zealand, Inc. to issue me a buyer created tax invoice for my bonus points.
